NEIGHBORHOOD FAMILY PRACTICE (NFP) IS A NETWORK OF SEVEN COMMUNITY HEALTH CENTERS ON THE WEST SIDE OF CLEVELAND DEDICATED TO PARTNERING WITH THE COMMUNITY FOR EVERYONE'S BEST HEALTH. NFP HAS MORE THAN 200 EMPLOYEES (MANY OF WHOM ARE BILINGUAL) INCLUDING 56 MEDICAL, DENTAL, PHARMACY AND BEHAVIORAL HEALTH PROVIDERS. NFP CURRENTLY PROVIDES SERVICES AT SEVEN COMMUNITY HEALTH CENTER LOCATIONS: RIDGE (3569 RIDGE RD.); DETROIT SHOREWAY (6412 FRANKLIN BLVD.); PURITAS (14625 PURITAS AVE.); TREMONT (2358 PROFESSOR AVE.) W. 117TH (11709 LORAIN AVE.); NORTHCOAST HEALTH (16110 DETROIT AVE) AND Ann B Reichsman (3545 RIDGE RD). ORAL HEALTH (DENTAL) SERVICES ARE PROVIDED AT THE RIDGE COMMUNITY HEALTH CE
TO PROVIDE PRIMARY,PREVENTIVE, MIDWIFERY AND DENTAL HEALTH CARE SERVICES TO THOSE LIVING ON CLEVELAND'S WEST SIDE, WHERE THERE IS LIMITED ACCESS TO HEALTH CARE.
Primary Medical Care - All seven neighborhood family practice community health center locations have received quality assurance recognition as patient centered medical homes that provide a high quality and personal approach to family medical care. Dedicated care teams take a collaborative approach to providing patient-centered care, ensuring a more personal and trusting relationship between the patient and their care team. Patient care teams, led by physicians and nurse practitioners, are supported by nurses, patient advocates, medical assistants and behavioral health staff. Patients of all ages and with all types of health insurance are welcome, and a sliding fee scale is provided for those without insurance.
Behavioral Health - Behavioral health services are integrated into the care team model at all of our health centers and are offered to established pediatric, adolescent and adult patients. This team approach ensures all patient concerns are treated, leading to better outcomes. Services provided include psychiatry, psychotherapy and medication assisted treatment. NFP has established strong relationships with other community organizations and providers to which patients are referred for specialty services.
MIDWIFERY - NFP'S CERTIFIED NURSE MIDWIVES CARE FOR WOMEN OF ALL AGES, PROVIDING A WIDE RANGE OF WOMEN'S HEALTH SERVICES. FOR THOSE WHO ARE EXPECTING, NFP'S NURSE MIDWIVES PROVIDE COMPLETE CARE BEFORE, DURING AND AFTER PREGNANCY. NFP IS AN APPROVED SITE FOR THE CENTERINGPREGNANCY PROGRAM OF THE CENTERING HEALTHCARE INSTITUTE.
