CommuniCare Health Centers is a private, non-profit health care organization providing comprehensive medical and dental services, substance abuse treatment, behavioral health services, and health education throughout Yolo County at four community clinic sites located in Davis, Woodland, Esparto, and West Sacramento. Since 1972, CommuniCare has been promoting healthy communities by providing affordable and culturally competent health care for those in need. CommuniCare Health Centers is a Federally Qualified Health Center (FQHC).
CommuniCare operates three primary care centers at carefully chosen locations to provide the best access to healthcare for Yolo county residents who cannot afford the full cost of healthcare and have no health insurance. Services include primary care, reproductive health, pediatric and adolescent services. Clinical facilities are located in Davis, West Sacramento, and Woodland. Visits totaled 51,050; total patients served was 16,613.
Perinatal Services - A full spectrum of pre and post natal care, births health education, nutrition counseling, and social work support for low-income and indigent women of Yolo county. The program is a certified California Perinatal Services Program (CPSP). Services are offered at five facilities in Davis, West Sacramento, Woodland, Winters and Dixon with births at Sutter Davis Hospital. Visits totaled 13,438 and births at Sutter Davis Hospital totaled over 500.
Behavioral Health Services is an outpatient substance abuse treatment program which offers medically assisted withdrawal and treatment to adults and adolescents involved with methamphetamine, opiates, and marijuana in groups and one on one counseling. Additionally, this department now offers life counseling services provided by licensed clinical social workers. Services are offered at three CommuniCare facilities: Davis, West Sacramento and Woodland. Visits totaled 50,948, serving over 4,923 patients.
