Other Programs The Solano Coalition Other Programs support activities that address outreach and enrollment for children, as well programs and services that increase access to health and wellness services for adults. These included, but are not limited to the following: Solano Kids Insurance Program (SKIP) helps Solano County children get the health coverage they need by enrolling children into medical, Healthy Families or the Healthy Kids Solano program. Parents receive additional outreach from SKIP staff to retain coverage and to use the health care system appropriately. Whole Person Care a case management and outreach program for the highest users of medical care in Solano County. SCBH works with community providers to provide the outreach, engagement, access to system resources, care planning, housing and comprehensive treatment to the homeless in our community for improved individual outcomes. Covered California - The Solano Coalition provides Covered California and other enrollment assistance free of charge at sites throughout Solano County, including Dixon, Fairfield, Suisun City, Vacaville and Vallejo. Coalition House - The Coalition House was a pilot project started in the fall of 2017 to provide housing to homeless Whole Person Care clients who had income but weren't able to rent due to adverse rental history or previous convictions. Additionally, the goal was to provide housing based on the Housing First model and to clients that were identified as low risk, needing fairly low level of service needs.

Transitional Care Program (TCP) - A respite program for homeless clients discharged from inpatient care. The TCP provides post-discharge medical care in a home in Vallejo and another in Fairfield. The program addresses the medical and psycho-social issues that often lead to continued homelessness including mental illness and substance abuse. Prior to discharge from the program, clients receive assistance in locating permanent housing, in signing up for health coverage and in establishing a medical home.

Health Effectiveness Data and Information Set (HEDIS) HEDIS is a comprehensive set of standardized performance measures designed to provide purchasers and consumers with the information they need for reliable comparison of health plan performance. HEDIS performance data can be used to identify opportunities for improvement, monitor the success of quality improvement initiatives, track improvement, and provide a set of measurement standards that allow comparison with other plans. The Solano Coalition supported Solano County health centers to improve their HEDIS clinical quality scores.
