Organizations Filed Purposes:
To provide direct service programs on behalf of frail, homebound seniors living in Fremont, Newark, and Union City, CA. That promote healthy lifestyle choices, support daily well-being and enable independent living for as long as safely possible.
Meals on Wheels-Provides over 102,000 home-delivered meals to frail, homebound seniors living in Fremont, Newark, and Union City, CA. The majority of these at-risk individuals live alone and often have no other daily contact other than their MOW Volunteer driver. Drivers bring a hot, nutritious meal and check on their well being enabling seniors to live independently in their own homes for as long as safely possible.
Falls prevention- provides a multi-faceted approach towards preventing falls including 1.) 12-week in-home exercise program, 2.)Home safety check, 3.)Minor home modifications and 4.)medications management review. This program is at no cost to homebound seniors
Friendly visitor program- matches community volunteers with lonely homebound seniors who spend a minimum of two hours together all in the name of friendship. Currently 150 seniors have been matched with a volunteer friend.
