Los Angeles Christian Health Centers is firmly committed to increasing access to healthcare to all homeless and low income residents of Los Angeles by providing responsible, quality, Christ-centered, comprehensive healthcare services.
During the year, LACHC served approximately 10,800 patients through 38,142 primary care medical visits. LACHC's comprehensive primary and prevention services included: episodic care, chronic disease management, HIV primary care, health screening, routine physicals, pharmacy, radiology and laboratory services. LACHC uses a service delivery model designed to be accessible to homeless patients, with 2 full-time clinics and 13 part-time sites that are integrated at homeless shelters, rehabilitation programs, drop-in centers, and mental health facilities. Specialty care is provided under established relationships and agreements.
Social Services staff provided case management and care coordination to approximately 986 vulnerable patients. Case management services are focused on housing for homeless persons, acquiring public benefits, addressing immediate needs such as food and transportation, and referring patients to appropriate resources in the community. Care coordination is designed to assist patients in navigating the complicated healthcare delivery system as efficiently as possible in order ot improve patient engagement in care and health outcomes.
LACHC provided dental services to approximately 3,000 patients through 9,236 visits during the year. Oral health services are provided to patients at five clinic sites. Services include dental examinations, x-rays, cleanings, deep cleansings, fillings, extractions, and dentures.
