bridge housing - a- greater self-determination - goal: 50% of families will accomplish no less than two goals in their service plan prior to successfully completing the programb- increased skills or income- goal: 70% of adults will maintain or increase their total income (from all sources) when they exit the programc- residential stability- goal: 50% of families will graduate to permanent housing after successfully completing the program outcome: 33% of our families exited to permanent housing (subsidized or unsubsidized).this year we served families with higher barriers, i.e. serious mental health issues and histories of long term abuse. we attribute this to a lower rate of moving families into permanent housing this fiscal year. these clients ended up staying in our program longer and needed more community wrap-around services, i.e. legal assistance and specialized housing referrals.outcome: 50% of our families accomplished no less than two goals on the individual service plansoutcome: 66% of our graduated adults maintain or increase their income
