Organizations Filed Purposes:
Health Outreach Partners mission is to build strong, effective, and sustainable grassroots health models by partnering with local community-based organizations across the country in order to improve the quality of life of low-income, vulnerable, and underserved populations.
TRAINING: During the 2018-2019 Fiscal Year, HOP delivered five (5) trainings a total of 94 participants, of whom 85 (90.4%) were from Health Centers or Primary Care Associations, representing 33 health center organizations. HOP also piloted one new curriculum during FY '18 19, providing Structural Competency training in collaboration with two different state PCAs, and revised the curriculum based on participant feedback.
INFORMATION SERVICES & RESOURCES: During the 2018-2019 Fiscal Year, Health Outreach Partners provided ten (10) webinars to 1,334 participants, 782 (58.6%) of whom were Community Health Center or Primary Care Association affiliated, representing 258 health center organizations. Additionally, HOP delivered eight (8) trainings or workshops at national and regional conferences to a total of 265 participants, of whom 181 (68.3%) were from Health Centers or Primary Care Associations, representing 71 health center organizations. A total of twenty (20) learning collaborative activities, which were web-based interactive trainings, were produced in partnership other National Cooperative Agreements and reached 674 health center and PCA staff, which was 98.27% of all 755 participants. In addition to the developed curricula, HOP published two (2) new resource: The Health Equity Starter Kit and the Leveraging Technology to Support Outreach: Innovation Outreach Practices from the Field report.
PROGRAM CONSULTATION: HOP provided six (9) client-centered consultations during the 2018-2019 Fiscal Year, with clients ranging from local governments, state PCAs, private foundations, and national non-profit organizations.
