Organizations Filed Purposes: The mission of the Center for Community Health and Well-Being is to improve the health and well-being of the Sacramento community and to ensure that its families and children have the best environment in which to grow and thrive.

In the 2016-2017 fiscal year, Center for Community Health & Well-Being, Inc. (CCHWB) continued to act as a leader in the Sacramento community, promoting health equity and providing quality support services to women at key stages of life: adolescence, pregnancy, and motherhood. The agency was excited to welcome a new executive director, Shannon Read, who assisted in strengthening organizational capacity, improving program processes, and developing a strategic plan for the 2017-2018 fiscal year and three years beyond. CCHWB continued to operate and increase productivity of two main programs for women, Black Mothers United and Girls on the Rise. Black Mothers United provides one-to-one support services for pregnant African American women including education, transportation to medical appointments, and referrals to community resources. During the 2016-2017 fiscal year, the program served a total of 114 pregnant women and welcomed the birth of 59 babies in the program. Girls on the Rise is a leadership, advocacy, and service program for high school-aged young women of color in South Sacramento. During 2016-2017, the program enrolled 12 youth members who met on a weekly basis throughout the school year. Meetings focused on development of leadership skills, peer mentor-ship, healthy lifestyles, and community activism. Youth members organized and hosted two community events as well as an annual leadership conference for young women at Sierra Health Foundation in June. The conference welcomed over 75 youth and community members, and informed attendees about self-care and relevant health topics. Finally, CCHWB was excited to host the 1st Annual Jazz & Wine Fundraiser in February 2017. The event was a success, welcoming approximately 100 guests, and helping CCHWB to secure additional revenue for the support and expansion of our programs.
