MEDICAL: SNAHC PROVIDES SERVICES TO INDIVIDUALS AND FAMILIES ACROSS ALL LIFECYCLES INCLUDING PRIMARY, PEDIATRIC, AND PREVENTIVE HEALTH CARE, IMMUNIZATIONS, MANAGEMENT OF ACUTE AND CHRONIC MEDICAL CONCERNS AND PROBLEMS (E.G.: DIABETES, HYPERTENSION, HEPATITIS C TREATMENT, TRANSGENDER HORMONE THERAPY), MEDICATION MONITORING, HEALTH ASSESSMENTS, HEALTH EDUCATION, ADVOCACY, AND SPECIALTY SERVICES INCLUDING OPTOMETRY, PODIATRY AND CHIROPRACTIC CARE.SNAHC IS A TITLE X GRANTEE THROUGH THE OFFICE OF POPULATION AFFAIRS AND A CERTIFIED FAMILY PLANNING, ACCESS, CARE, AND TREATMENT (FAMILY PACT) PROVIDER THIS GRANT ALLOWS SNAHC TO PROVIDE FAMILY PLANNING AND REPRODUCTIVE HEALTH SERVICES TO ITS PATIENTS, INCLUDING STD TESTING, CONTRACEPTION, CLINICAL AND PREVENTIVE SERVICES TO MAINTAIN REPRODUCTIVE HEALTH, AND INDIVIDUAL REPRODUCTIVE HEALTH EDUCATION AND COUNSELING.

DENTAL: SNAHC PROVIDES PREVENTIVE, RESTORATIVE, AND EMERGENCY DENTAL SERVICES ON SITE WITH 14 DENTAL OPERATORIES. SERVICES INCLUDE GENERAL DENTISTRY FOR ADULTS AND CHILDREN, EXAMS AND X-RAYS, EMERGENCY SERVICES, FILLINGS AND EXTRACTIONS, CLEANINGS, SEALANTS, FLUORIDE TREATMENT, REFERRALS, FULL AND PARTIAL DENTURES, ROOT CANALS, ORAL SURGERY, ANXIETY REDUCTION TECHNIQUES, AND PATIENT EDUCATION.

BEHAVIORAL HEALTH: SNAHC PROVIDES COMPREHENSIVE BEHAVIORAL HEALTH SERVICES TO INDIVIDUALS ACROSS THE LIFESPAN AND OUR INTEGRATED MODEL IS INCLUSIVE OF PREVENTION AND EARLY INTERVENTION, TREATMENT, AND AFTERCARE. PREVENTION AND EARLY INTERVENTION SERVICES INCLUDE: UNIVERSALLY SCREENING FOR DEPRESSION, ALCOHOL OR SUBSTANCE ABUSE, AND DOMESTIC VIOLENCE ACROSS SERVICE LINES, AND ROUTINE RISK ASSESSMENTS DURING INTAKE WITH AN INTEGRATED CARE COORDINATOR, AND WARM HAND-OFFS ACROSS SERVICE LINES. TREATMENT SERVICES INCLUDE: INDIVIDUAL, CHILD, AND ADOLESCENT COUNSELING (THERAPY) USING EVIDENCE-BASED PRACTICES, ALCOHOL AND OTHER DRUG (AOD) COUNSELING, PSYCHIATRY, AND REFERRALS FOR SERVICES OUT OF SCOPE, INCLUDING RESIDENTIAL TREATMENT AND/OR SUBSTANCE DETOXIFICATION. AFTERCARE SERVICES INCLUDE MANY SELF-MANAGEMENT CLASSES AND SUPPORT GROUPS SUCH AS PARENTING CLASSES, TWO SPIRIT CIRCLES, AND RED ROAD SUPPORT GROUPS.
