Organizations Filed Purposes: TO TRANSFORM THE LIVES OF THOSE LIVING WITH MENTAL HEALTH CHALLENGES, SUBSTANCE USE DISORDERS, AND HOMELESSNESS BY SUPPORTING THEIR INDEPENDENCE, PROMOTING THEIR HOUSING STABILITY, AND EMPOWERING THEM TO MAKE POSITIVE CHANGES IN THEIR LIVES.

TRANSFORMING AND EMPOWERING THE LIVES OF PEOPLE WITH MENTAL ILLNESS BY SUPPORTING INDEPENDENCE AND PREVENTING HOMELESSNESS.

RESIDENTIAL SERVICES INCLUDE INTERIM, AND PERMANENT SUPPORTED HOUSING FOR INDIVIDUALS WITH PSYCHIATRIC DISABILITIES. SPECIALIZED HOUSING PROGRAMS ARE AVAILABLE FOR INDIVIDUALS WITH SECONDARY CHALLENGES INCLUDING HOMELESSNESS, AND/OR SUBTANCE ABUSE. ADDITIONALLY, CLIENTS OF OUR RESIDENTIAL PROGRAMS HAVE BEEN TRAINED AND SUPPORTED TO STAFF A "WARM-LINE" TO SUPPORT OTHER CLIENTS, AND HAVE PARTICIPATED IN OTHER COMMUNITY VOLUNTEER EFFORTS. TLCS PROVIDES RESIDENTIAL SERVICES TO 177 PEOPLE. TLCS RESIDENTIAL CLIENTS ARE ALSO ELIGIBLE TO PARTICIPATE IN THE TLCS CLUBHOUSE. THE "CLUBHOUSE" PROGRAM WAS CONCEIVED IN 2015 TO SUPPORT OUR CLIENTS TO ACHIEVE THEIR PERSONAL GOALS. MANY OF OUR CLUBHOUSE MEMBERS HAVE EXPRESSED AN INTEREST IN ENTERING OR RETURNING TO GAINFUL EMPLOYMENT. SEVERAL ARE PURSUING PEER SUPPORT CERTIFICATION, HUMAN SERVICES EDUCATION OR WORKING WITH THE DEPARTMENT OF REHABILITATION OR GOODWILL INDUSTRIES TO OBTAIN JOB SKILLS AND TRAINING. THE CLUBHOUSE IS OPEN 5 DAYS PER WEEK, 6 HOURS PER DAY AND OFFERS A VARIETY OF WORK-ORDERED DAY ACTIVITIES AS WELL AS OPPORTUNITIES FOR SOCIAL CONNECTEDNESS.

CASE MANAGEMENT SERVICES ARE PROVIDED TO ADULTS WITH PSYCHIATRIC DISABILITIES, SOME OF WHOM ARE EXPERIENCING HOMELESSNESS. THESE SERVICES INCLUDE CRISIS INTERVENTION, INFORMATION AND REFERRALS, HOUSING ASSISTANCE, BENEFITS ADVOCACY, EMPLOYMENT ASSISTANCE, SUPPORT TO APPOINTMENTS, AND COORDINATED PSYCHIATRIC CARE AND THERAPY.

IN 2015, TLCS IN COLLABORATION WITH SACRAMENTO COUNTY DEPARTMENT OF BEHAVIORAL HEALTH SERVICES LAUNCHED THE TRIAGE NAVIGATOR PROGRAM. TLCS NAVIGATORS ARE EMBEDDED WITHIN LOCAL HOSPITAL EMERGENCY ROOMS INCLUDING SUTTER GENERAL, MERCY GENERAL, METHODIST HOSPITAL, UC DAVIS, MERCY SAN JUAN AND KAISER SOUTH. IN ADDITION, STAFF ARE LOCATED AT THE LOAVES & FISHES CAMPUS AND RESPOND TO THE MAIN JAIL UPON RELEASE. THESE NAVIGATORS RECEIVE REFERRALS FROM PARTNER STAFF TO ENGAGE WITH AND CREATE LINKAGES FOR PEOPLE EXPERIENCING A MENTAL HEALTH CRISIS THAT HAS CREATED A SERIOUS FUNCTIONAL IMPAIRMENT. STAFF WORK WITH ALL AGES WITH THE INTENTION OF CREATING AN ACTION PLAN AND WARM HANDOFF TO PEER NAVIGATORS THAT WILL FOLLOW UP TO ENSURE THAT SUCCESSFUL LINKAGES ARE MADE. THE GOAL IS TO PREVENT UNNECESSARY, COSTLY AND INHUMANE INTERACTIONS WITH LAW ENFORCEMENT AND HOSPITAL EMERGENCY DEPARTMENTS.
