Organizations Filed Purposes: The mission of TFCF is to empower disenfranchised low income individuals and families by building leadership through evidence-based programs and housing to create self-sufficiency and thriving communities.

Housing, Counseling, and training for low income communities.

TFCF has ended homelessness for 287 families and 92 children ages 0-5We have reunified 299 children from foster care back with their moms with our various housing programs78 women have reached self-sufficiency through our social economic mobility platformProvided re-entry services for 47 women from incarceration back into the community Transitioned 149 families into permanent affordable housing Increased our housing capacity to the serve an additional 15 families We currently operate in both Northern and Southern CaliforniaSouthern CA53 families received a total of 6,243 shelter nights in our Emergency Shelters and received a total of 149,832 hours of Case Management, Childcare, Transportation, Life Skills, Counseling, Financial Education and Leadership Development. Of those clients, 27 women and 14 children transitioned into affordable housing becoming self-sufficient and alleviating housing insecurity. In total 6 children were reunified with their moms from foster care bringing our total to 298.Other supportive services provided included: 332 Case Management sessions 1,914 hours of Independent Living Skills Workshops 1,419 hours of Women's Wellness Sessions 272 workshops on Relapse Prevention 660 hours of Mommy & Me Child Bonding Sessions 1,056 hours of Financial Education and Money Management Sessions 2,146 hours of Parenting Education Workshops 1,665 hours of Children's Therapy Sessions 164 hours of family healthcare visits 81 Work to Win/Employment Readiness Workshops 1822 Transports to medical, educational, and therapeutic appointmentsNorthern CABrighter Futures - Our first Bay Area project served a total of 17 families during this fiscal year who received a total of: 872 Shelter nights in our Emergency Shelter 11,476 Hours of Case Management 87 Transportation Assistance 476 Life Skills Workshops 315 Financial Literacy and Money Management classes 137 Leadership Development and Civic Engagement trainings Of those clients, 7 women and 9 children transitioned into co-abode housing becoming self-sufficient and alleviating housing insecurity. In total 1 child was reunified with her mom from foster care bringing our total to 298.Positive Family Futures Project 2-GEN (Two-Generations) Children who suffer from trauma, family separation, exposure to domestic and other forms of violence, and other special needs are provided with their own Social Services Case Manager who conducts a full assessment to provide appropriate case management and therapeutic interventions to end generational cycles of violence, poverty and low educational attainment. Fifty-eight children succeeded in reaching over 80% of their development goals. All children demonstrated increases across 5 social and emotional indicators. The Rights, Equality and Law Project (REAL Project) Utilizes the expertise of a Root and Rebound Attorney, who is embedded into our offices and provides direct legal advocacy, education, and support to relieve our women from barriers related to family reunification, employment discrimination, fines and fees, and higher educational attainment. Navigating systems of care to have meaningful impact for our families requires persistent lawyering. 70 families were screened and assessed to identify any legal barriers fees and fines to provide expungement support and family reunification. 90% of those clients demonstrated increased access to services including employment and educational attainment.Positive Futures I - Asset Development and Wealth Creation Project designed to assist women with economic mobility by providing a pathway from poverty to prosperity. Key Strategies include: Employment Development, Entrepreneurship, Financial Education and Access Of the 31 formerly incarcerated women of color have completed the program 4 started their own businesses, 6 enrolled in higher education, 11 gained employment with a 3-year plan for upward mobility and all established a savings account. Additionally, all women received mentorship and access to our other supportive services.Positive Futures II -Reentry Program for women diagnosed with mental health and/or substance abuse returning to 5 specific cities within San Bernardino County from state prisons/jails. This 5-year project funded by SAMHSA allows us to serve 220 women (44 per yr.) with evidence-based treatment, housings, education, and peer mentorship support. TFCF is currently at 100% percent capacity, exceeding our expected goals for year 2 with 100% elimination of recidivism.Homes of Hope Permanent-Supportive Housing for chronically homeless women and those with children. Funded by HUD, this project ensures that women and their children receive preventative care, educational assurances and alleviates substantial cost to taxpayers by alleviating the utilization of emergency services such as hospitals, ambulances, etc. In total 11, women and 21 children were served yielding 95% retention rate. TFCF's Licensed Drug and Alcohol program served 45 women diagnosed with substance abuse disorders with outpatient drug treatment. As part of rehabilitation, our clients attended 274 classes of drug and alcohol counseling sessions 4,752 hours of twelve-step meetings, and 726 hours of Leadership Development training workshops. To ensure that we support families committing to changing their way of life and a drug free lifestyle, we conducted 317 random drug tests throughout this period. TFCF issued 32 certificates of completionWellness is Stabilized Housing 4 Kids (WISH4Kids) program provides community wide assistance to housing insecure families by prevention evictions, homelessness, and food insecurity. In total, we assisted 25 families with interventions such rental and utility assistance. COVID-Relief 27 families received eviction prevention services, 18 women exiting state prison received 14-day quarantine stays in a hotel post-release. Each client was provided with protective equipment, food cards, mentorship, and resources to access additional support. Additionally, 26 families were stabilized in housing with food and utility assistance.
