Organizations Filed Purposes: COMMUNITY CLINICS HEALTH NETWORK DBA HEALTH QUALITY PARTNERS OF SOUTHERN CALIFORNIA ("HQP") WAS FOUNDED IN 1993 AND INCORPORATED IN 1997 AS A 501(C)(3) NONPROFIT PUBLIC BENEFIT CORPORATION.HQP IMPLEMENTS PROGRESSIVE, COLLABORATIVE PROGRAMS WITH HEALTH CENTERS AND OTHER SAFETY-NET PARTNERS, RESULTING IN IMPROVED ACCESS TO CARE, QUALITY HEALTH OUTCOMES AND OPERATIONAL EFFICIENCIES.

HQP AIMS TO IMPROVE HEALTH OUTCOMES AMONG THE SAFETY NET POPULATION BY IMPLEMENTING PROGRESSIVE AND COLLABORATIVE PROGRAMS FOR COMMUNITY HEALTH CENTERS AND OTHER SAFETY NET PARTNERS.

IN FISCAL YEAR 2019-2020, OHI PROVIDERS SCREENED 12,877 CHILDREN AND 1,960 PREGNANT WOMEN FOR ORAL HEALTH NEEDS AND PERFORMED DENTAL EXAMS FOR 12,792 CHILDREN AND 1,582 PREGNANT WOMEN. A TOTAL OF 11,019 CHILDREN AND 1,784 PREGNANT WOMEN RECEIVED TREATMENT FOR IDENTIFIED ORAL HEALTH NEEDS, INCLUDING THOSE CONSIDERED TO BE AT HIGH-RISK FOR DENTAL DISEASE ADDITIONALLY, ORAL HEALTH EDUCATION WAS PROVIDED IN A CLINIC OR COMMUNITY SETTING TO A TOTAL OF 4,726 PREGNANT WOMEN AND PARENTS OF CHILDREN AGES ZERO THROUGH FIVE.

IN FISCAL YEAR 2019-2020, THE BEHAVIORAL HEALTH AND PRIMARY CARE INTEGRATION PROJECT SERVED MORE PEOPLE (888) THAN IN ANY PREVIOUS YEAR OTHER THAN FY 18-19, SUBSTANTIATING ITS ONGOING IMPORTANCE TO THE COMMUNITY. THE PROGRAM ALSO HOSTED THE 10TH ANNUAL INTEGRATION SUMMIT WHICH WAS ATTENDED BY LEADERSHIP AND CLINICIANS FROM THE PRIMARY CARE, MENTAL HEALTH AND SUD TREATMENT SYSTEMS OF CARE AND WAS RATED EXCELLENT OR GOOD BY 97% OF THOSE ATTENDEES.

THE SOCAL OPTIMIZATION OF SBIRT (SOS) PROJECT, WHICH BEGAN IN NOVEMBER 2018 AND RUNS FOR FIVE YEARS, UNIFORMLY OPTIMIZES AND STANDARDIZES SCREENING, BRIEF INTERVENTION AND REFERRAL TO TREATMENT (SBIRT) SERVICES AT FQHCS IN SAN DIEGO, RIVERSIDE AND IMPERIAL COUNTIES TO PREVENT AND REDUCE ALCOHOL AND OTHER SUBSTANCE MISUSE AMONG ADOLESCENTS (AGES 12-17) AND ADULTS (AGES 18+) IN PRIMARY CARE AND COMMUNITY HEALTH SETTINGS. THIS PROJECT IS FUNDED BY THE SUBSTANCE ABUSE AND MENTAL HEALTH SERVICES ADMINISTRATION.
