MHF HAS RAISED OVER $2.5 MILLION DOLLARS TO DISRUPT BUSINESS AS USUAL IN THE FIGHT AGAINST CHRONIC DISEASES BY REINVESTING COST SAVINGS, CONTRACTS WITH HEALTH PROVIDERS, SOCIAL HEALTH FRANCHISING AND COMMUNITY-BASED ENTREPRENEURIAL INITIATIVES. OUR MISSION IS TO BRING HEALTH JUSTICE AND WELLNESS TO THE MULTICULTURAL COMMUNITIES OF SAN DIEGO COUNTY BY FOCUSING RESOURCES ON THE MOST VULNERABLE POPULATIONS WITH COMMUNITY-BASED WELLNESS STRATEGIES, SOCIAL-CLINICAL INTERVENTIONS, AND RESEARCH THAT WILL LEAD TO THE ELIMINATION OF RACIAL AND ETHNIC HEALTH DISPARITIES. MHF ACHIEVES THESE GOALS USING THE FOLLOWING INITIATIVES:
OUR MISSION IS TO BRING HEALTH JUSTICE AND WELLNESS TO MULTICULTURAL, MEDICALLY UNDERSERVED COMMUNITIES USING WELLNESS STRATEGIES, AT-HOME SOCIAL-CLINICAL INTERVENTIONS, AND RESEARCH TO ELIMINATE RACIAL AND ETHNIC HEALTH DISPARITIES.
SINCE 2012, THE PATIENT HEALTH IMPROVEMENT (PHII) MODEL SERVES OVER 650 ETHNICALLY DIVERSE, CHRONICALLY ILL ADULTS AGES 18 TO 85 RESIDING IN SOUTHEASTERN SAN DIEGO WHERE CHRONIC DISEASES ACCOUNT FOR 57% OF DEATHS AND RESIDENTS ARE SUPER-UTILIZERS OF THE HEALTHCARE SYSTEM. IT ACHIEVES THE TRIPLE AIM WITH 90% CUSTOMER SATISFACTION, 67% DECREASE IN HOSPITAL ADMISSIONS, AN ESTIMATED $16 MILLION SAVED WITH A 5:1 RETURN ON INVESTMENT. AT HOME, THE CLINICAL, SOCIAL AND BEHAVIORAL CARE TEAM WORKS CLOSELY WITH PATIENTS FOR SIX MONTHS TO PROVIDE ACCESS TO QUALITY CARE AND SOCIAL DETERMINATES OF HEALTH SOLUTIONS LIKE NUTRITIONAL FOOD AND HOUSING. PHII BUILDS UPON DR. JEFFREY BRENNER'S THEORY OF HOT SPOTTING FOR INTEGRATING COMPREHENSIVE MEDICAL, SOCIAL AND MENTAL HEALTH SERVICES INTO THE COMMUNITY HEALTH NEEDS OF ETHNICALLY DIVERSE PATIENTS. THE CENTER FOR MEDICAID AND MEDICARE ACKNOWLEDGED PHIII SUSTAINABLE BUSINESS MODEL USING FEE-FOR-SERVICE CONTRACTS WITH HEALTH PROVIDERS.
THE HEALTH EQUITY ACTION THINK TANK (HEATT) SERVES AS A COLLABORATIVE CENTRAL VOICE FOR SHAPING PUBLIC POLICIES AND SPEARHEADING COMMUNITY ACTION TO AFFECT POSITIVE CHANGES IN THE HEALTH OF MULTICULTURAL NEIGHBORHOODS IN SAN DIEGO COUNTY.
THE PREVENTION ALLIANCE - THE MULTICULTURAL HEALTH FOUNDATION WAS AWARDED $1 MILLION DOLLARS FROM THE ALLIANCE HEALTHCARE FOUNDATION I2 INNOVATIONS GRANT TO ESTABLISH THE PREVENTION ALLIANCE (PA). THE PA PIONEERS SAN DIEGO'S FIRST SOCIAL HEALTH FRANCHISING MODEL (SHF) TO ACCELERATE REPLICATION OF PREVENTION SERVICES IN CLINICS, SCHOOLS, EMPLOYER AND COMMUNITY SITES. AS A RESULT, PREVENTION WORKSHOPS NOW OPERATE AT EMPLOYER SITES, CLINICS, AND COMMUNITY ORGANIZATIONS. THESE IN-PERSON WORKSHOPS GUIDE PREDIABETICS AGE 17 TO 87 THROUGH THE CDC'S NATIONAL DIABETES PREVENTION PROGRAM AND ARE TAUGHT MULTIPLE FOREIGN LANGUAGES.
