SENIORS AND ADULTS WITH DISABILITIES WHO VISIT OUR SITES EXPERIENCE DECREASED RISKS OF ISOLATION AND LONELINESS AND ENJOY THE POSITIVE EFFECTS OF SOCIAL INTEGRATION AND IMPROVED QUALITY OF LIFE. AS THE LARGEST CONGREGATE MEAL PROVIDER FOR THESE POPULATIONS IN SAN FRANCISCO, PROJECT OPEN HAND HAD TO QUICKLY MAKE CHANGES AND ADJUSTMENTS TO RESPOND TO THE COVID-19 PANDEMIC TO ENSURE CLIENTS WERE STILL RECEIVING NECESSARILY NUTRITIONAL RESOURCES WHILE ALSO BEING SAFE AND SHELTERING IN PLACE AS HIGH RISK POPULATIONS. SERVICES WERE UNINTERRUPTED BUT CHANGED TO A MODEL THAT FOCUSED PRIMARILY ON ADDRESSING FOOD INSECURITY FOR HIGHLY VULNERABLE POPULATIONS. INCREASED DEMAND WAS MET THROUGH A SAFE FOOD TAKEAWAY MODEL.
WELLNESS PROGRAM - THE WELLNESS PROGRAM OFFERS A MEDICALLY-TAILORED NUTRITION INTERVENTION TO SUPPORT OUR SICK AND VULNERABLE NEIGHBORS FACING CRITICAL ILLNESSES. SERVICES INCLUDE NUTRIENT-RICH MEALS, FRESH GROCERIES, AND NUTRITION COUNSELING AND EDUCATION WITH OUR TEAM OF REGISTERED DIETITIANS. CLIENTS CAN SHOP FOR FRESH, HEALTHY GROCERIES AT OUR TWO GROCERY CENTERS LOCATED IN SAN FRANCISCO AND ALAMEDA COUNTY. HOME-DELIVERED MEALS ARE AVAILABLE TO CLIENTS WITH MOBILITY ISSUES. DURING MARCH OF 2020, THE WELLNESS PROGRAM ALSO BEGAN DELIVERING GROCERIES TO CLIENTS TO HELP SUPPORT CLIENTS SHELTERING IN PLACE DUE TO THE COVID-19 PANDEMIC.
PROJECT OPEN HAND HAS BEEN ADVOCATING FOR THE EXPANSION OF MEDICALLY TAILORED MEAL SERVICES TO THOSE LIVING WITH CHRONIC DISEASE AT THE LOCAL, STATE AND NATIONAL LEVEL AND ARE PART OF THE CALIFORNIA FOOD IS MEDICINE COALITION.SINCE 2017, PROJECT OPEN HAND HAS LEAD A FIRST-OF-ITS-KIND MEDICAL NUTRITION PILOT PROGRAM ACROSS THE STATE OF CALIFORNIA WITH OVERSIGHT FROM THE CALIFORNIA DEPARTMENT OF HEALTH CARE SERVICES (DHCS). THE MEDI-CAL MEDICALLY TAILORED MEALS (MTM) PILOT PROGRAM IS A MEDICAL NUTRITION INTERVENTION FOR MEDI-CAL BENEFICIARIES WITH A DIAGNOSIS OF CONGESTIVE HEART FAILURE. AIMED AT IMPROVING HEALTH OUTCOMES AND REDUCING HEALTHCARE COSTS, THE MEDI-CAL BENEFIT INCLUDES COMPLETE NUTRITION OF THREE MEALS PER DAY, FOR 12 WEEKS AND FOUR MEDICAL NUTRITION THERAPY SESSIONS DURING THE INTERVENTION, AT NO COST TO THE PATIENT. WE HAVE FURTHER EXPANDED OUR PARTNERSHIPS WITH HEALTH PLANS THOUGHOUT CALIFORNIA AND MANAGED CARE PLANS IN SAN FRANCISCO AND ALAMEDA COUNTY TO PROVIDE HEALTH INTERVENTIONS THAT OFFER MEDICALLY TAILORED MEALS AND MEDICAL NUTRITION THERAPY AND/OR NUTRITION EDUCATION.
