SCHOOL HEALTH CLINICS OF SANTA CLARA COUNTY IS COMMITTED TO KEEPING KIDS HEALTHY AND IN SCHOOL BY PROVIDING HIGH QUALITY, EASILY ACCESSIBLE PRIMARY MEDICAL CARE AND PREVENTIVE HEALTH SERVICES TO LOW-INCOME CHILDREN FROM INFANCY TO AGE NINETEEN.

DURING THE YEAR ENDED JUNE 30, 2019, THE ORGANIZATION CONTINUED TO STRIVE TO PROVIDE HIGH-QUALITY, AFFORDABLE HEALTH CARE TO LOW-INCOME FAMILIES IN SANTA CLARA COUNTY. SCHOOL HEALTH CLINICS PROVIDED 14,815 HEALTH CARE VISITS TO 5,795 PATIENTS ACROSS ALL FIVE SCHOOL HEALTH CLINIC SITES. THE ORGANIZATION TOOK PART IN AN OPERATIONAL SITE VISIT BY THE HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA) IN JULY 2018. PREPARATION INCLUDED AN UPDATED OF CLINICAL, FINANCIAL, AND MANAGEMENT POLICIES AND PROCEDURES AS WELL AS VERIFICATION THAT THE ORGANIZATION WAS MEETING ALL 19 PROGRAM REQUIREMENTS AS A FEDERALLY QUALIFIED HEALTH CENTER (FQHC) AND A HRSA GRANTEE. DURING THE 2018 AUDIT, IT WAS DISCOVERED THAT THE ORGANIZATION HAD SOME FINDINGS RELATED TO PROGRAM REQUIREMENTS, HOWEVER, THE ORGANIZATION LEADERSHIP WORKED QUICKLY TO REMEDY FINDINGS AND WAS FOUND TO BE IN COMPLIANCE WITH ALL PROGRAM REQUIREMENTS SOON AFTER AND ALL CONDITIONS WERE REMOVED. THE ORGANIZATION'S LEADERSHIP SPENT MOST OF FISCAL YEAR 2016 ENSURING COMPLIANCE WITH ALL 19 PROGRAM REQUIREMENTS IN ADVANCE OF THE SERVICE AREA COMPETITION APPLICATION RENEWAL THAT WOULD BE DUE LATER IN 2016 FOR CONTINUED FEDERAL FUNDING FROM HRSA.THE BOARD OF DIRECTORS AND STAFF OF THE ORGANIZATION TOOK PART IN SEVERAL MEETINGS, INCLUDING A DAY-LONG RETREAT IN DECEMBER 2018 TO FORMULATE THE 2019-2021 ORGANIZATION STRATEGIC PLAN. THE THREE-YEAR PLAN INCLUDES A FOCUS ON THE FIVE FOLLOWING STRATEGIES:1. SUPPORT AND RETAIN A HIGH-QUALITY WORKFORCE2. ENSURE THE ORGANIZATION IS FINANCIALLY STABLE3. INCREASE THE ORGANIZATION'S VISIBILITY AND PARTNERSHIPS4. IMPROVE QUALITY5. OPTIMIZE AND EXPAND SERVICES
