Health Projects Center exists to address the health and human needs of individuals, families and communities of California's Central Coast by developing and implementing high quality programs and strategies.
Multi-Purpose Senior Services Program (MSSP) - The Santa Cruz and Monterey Multipurpose Senior Services Program (MSSP) provided over 18,000 hours of complex -care management for 413 medically frail elderly clients who are certifiable for placement in a skilled nursing facility but wish to remain in the community. Our team of social workers and nurses arrange for and monitor the use of community services to prevent or delay premature institutional placement of these medically frail clients.
Del Mar Caregiver Resource Center (DMCRC) - DMCRC provides free supportive services and programs for family caregivers (any loved one) caring people with cognitive impairments. DMCRC serves Monterey, San Benito and Santa Cruz Counties. Caregiver services provided over the year include: 2,000 hours of one-one-one family consultations for 349 family caregivers, 3,753 hours of respite care provided, 212 hours of training and workshops delivered, 81 hours of support groups facilitated.
Central Coast Area Health Education Center (AHEC) - Central Coast AHEC works to improve the availability and quality of health professionals serving underserved communities. Over the past year, HPC supported training and more than 2,000 hours of community-based experiences for 96 students enrolled as nursing students and 24 family medicine residents.
