Organizations Filed Purposes: THE HEALTH TRUST AIMS TO IMPROVE HEALTH AND QUALITY OF LIFE FOR RESIDENTS OF THE GREATER PAJARO VALLEY BY PROVIDING HEALTH PROMOTION SERVICES AND DIABETES SELF-MANAGEMENT EDUCATION. ADDITIONALLY, WE CONVENE CROSS-SECTOR STAKEHOLDERS TO ALIGN RESOURCES TO REALIZE A SHARED VISION OF HEALTH AND PROVIDE GRANTS AND ALLIED SUPPORT TO LOCAL GROUPS ENSURING ACCESS TO HEALTH PROMOTION AND DISEASE PREVENTION PROGRAMS AS WELL AS QUALITY, CULTURALLY COMPETENT WHOLE PERSON HEALTHCARE.

DIABETES HEALTH CENTER (DHC) - The Health Trust operates the Diabetes Health Center (DHC), a community-based health education and diabetes care program that provided culturally responsive, empowering disease self-management and health promotion education in 2,952 individual sessions and 21 workplace wellness, job site education and community outreach events this year benefiting 1,250 individuals.

GRANT MAKING - The Health Trust awarded 22 grants totaling $222,000 to local nonprofits working to promote active living & healthy eating as cornerstones of health. Grant funds also strengthened nonprofit organizations that provide oral, physical and mental health care for those facing the greatest obstacles to care. The health trust awarded 23 grants totaling $20,000 to 16 students to pursue careers in the health professions.

LEADERSHIP - The Health Trust builds, sustains and leverages partnerships with peer nonprofits promoting health and well-being in the tri-county region encompassing parts of Santa Cruz, Monterey and San Benito Counties. We convene the Pajaro Valley Food, Farming & Health Policy Council, a cross-sector group working to create a local environment where the healthy choice is the easy choice for every resident. We are an active contributor of the Health Improvement Partnership of Santa Cruz County, a roundtable of healthcare leaders implementing collaborative solutions to priority issues affecting health equity and outcomes, especially for marginalized populations.
