To help provide for the basic needs of the homeless and precariously housed in Yuba and Sutter counties, including, but not limited to, feeding and providing shelter for the homeless. To address issues affecting the homeless in the community and work on building partnerships and coordination of services among agencies/groups who serve the homeless population in the region.
Implemented coordinated entry at two locations, one in Sutter and one in Yuba County. The coordinated entry system identifies, assesses, refers and connects homeless individuals and/or families to housing and assistance based on each individual's strengths and needs. The goal of the coordinated entry system is to prioritize available assistance to the most vulnerable in the community. 750 unduplicated individulas residing in Sutter and Yuba Counties have been served.
