Adolescent Pregnancy Prevention Program (Y Club)5,038 youth, age 12 to 18, participated in this after school pregnancy prevention program. Along with nightly activities, the youth also spent time at a local nursing home once a month throughout the year.Anna B. Adoption staff worked with 3 families seeking to adopt a baby in the future.Adult Residential TreatmentFive homes, four in Dubuque and one in Iowa City, functioned to assist psychiatrically disabled adults. 5 individuals lived for various amounts of time at the Iowa City house and 33 in the Dubuque houses.Anti-Tobacco Program4,455 participants were serviced in tobacco diversion, a program for youth cited with underage tobacco use.Mentor-Dubuque182 school and community-based matches between adult and children occurred. Mentor Dubuque focuses on providing service activities for the matches to participate in together.Emergency Shelter Care9 youth in crisis family situations, in danger of bodily harm, at risk of physical and sexual abuse or neglect or runaways received support at the Emergency Shelter.Family Centered Services26 clients were seen in the following programs: Supervised Apartment Living, Community based Remedial Services, Drug testing, Truancy, Family Support, Early Intervention (school & juvenile court).Health and Professional Health Clinic812 women and men were seen in the clinic; 2,131 in the HIV program. Hillcrest RCF and PMI201 adults were served at this residential rehabilitation facility for psychiatrically disabled adults. 70 moved to another level of care; one was re-admitted to the facility.Hillcrest Mental Health CentersDubuque Co. center and Asbury location saw a total of 6,452 clients receiving psychiatry and mental health counseling in Dubuque and 414 clients at the Solution Mental Health Center in Dubuque. 846 individuals received services at our Jackson County mental health program in Maquoketa, 67 at our Jones County program in Monticello, 598 at our Washington County and SE Iowa office 453 at our Henry County office and 388 in our Louisa County office.Subacute 132 clients receiving psychiatry and mental health counseling and housing.Hillcrest-Mercy Maternal Health ProgramThe Maternal Health program served 152 women in Dubuque, Delaware, Jones, Clinton counties.Hillcrest Supported LivingThe Supported Living program worked with 170 individuals in Dubuque, 23 Cedar Rapids and 42 Iowa City. Hillcrest Intergraded Health HomeThe IHH program worked with 1,279 individuals. Crisis StabilizationCrisis Stabilization program worked with 5 individuals.Mobile Crisis OutreachMobile Crisis Outreach program worked with 401 individuals.Homeless Outreach185 individuals with mental health issues received services through the Homeless Outreach program. Transitional Apartment Living5 participated in the supervised apartment living program, learning job skills, budgeting, daily living skills and receiving check-up from staff.Transitional Housing for Homeless Families1 family lived in the apartments. Wellness Center51 individuals received peer-driven, support-based services in recovery from mental illness, addictions, physical injury/illness and other life challenges. WIC ProgramThe WIC locations, Clinton and Dubuque, served 4,590 individuals. In 2020: Services were provided to individuals in 53 Iowa counties, 3 states (including the District of Columbia) for a total of 30,648 people served.
