Organizations Filed Purposes: WESLEY COMMUNITY SERVICES, INC. IS FOCUSED ON PROVIDING SERVICES TO OLDER ADULTS AND THOSE OF LOW INCOME. IT WAS ESTABLISHED IN 2005 AS A FAITH-BASED NON-PROFIT ORGANIZATION DEDICATED TO PROVIDING SERVICES TO PEOPLE OF ALL FAITHS IN A CHRISTIAN CARING WAY, SUPPORTING INDEPENDENCE AND AN ACTIVE LIFESTYLE. THE MISSION OF WESLEY COMMUNITY SERVICES, INC. IS TO PROVIDE IN-HOME SUPPORTIVE ASSISTANCE, MEALS AND COMPANIONSHIP FOR OLDER ADULTS REGARDLESS OF THEIR FINANCIAL MEANS. THESE SERVICES ARE CRITICAL AND FUNDAMENTAL TO ASSIST THEM IN REMAINING INDEPENDENT AND IN THEIR OWN HOMES.

TO PROVIDE OLDER ADULTS IN-HOME SUPPORTIVE HEALTH AND WELLNESS ASSISTANCE, REGARDLESS OF THEIR MEANS

WCS WAS ESTABLISHED IN 2005 WITH THE FOCUS OF PROVIDING SERVICES TO OLDER ADULTS AND THE INDIGENT REGARDLESS OF FINANCIAL MEANS THROUGH IN-HOME SUPPORT, ASSISTANCE, MEALS, AND COMPANIONSHIP. WESLEYLIFE IS THE SOLE MEMBER OF WCS AND AN AFFILIATE COMPANY, WESLEY RETIREMENT SERVICES, INC. (WRS), WHICH PROVIDES HOUSING, HEALTH CARE, AND SUPPORT SERVICES TO OLDER ADULTS. OUR MISSION IS GUIDED BY CHRISTIAN COMPASSION, TO SUPPORT THE INDEPENDENCE, HEALTH, AND WELL-BEING OF OLDER ADULTS WHEREVER THEY CALL HOME. AS TRENDS IN RETIREMENT LIVING CONTINUE TO CHANGE, WE HAVE RESPONDED WITH AN ARRAY OF SERVICES THAT PROMOTE AND DELIVER FRIENDSHIP AND FELLOWSHIP, A SLATE OF MEANINGFUL ACTIVITIES, LIFELONG LEARNING, AND COMPREHENSIVE HEALTH AND WELLNESS PROGRAMS TO ACCOMODATE THE EXPECTATIONS OF OUR CUSTOMERS. WCS IS A 501(C)(3) NON-PROFIT ORGANIZATION THAT PROVIDES HOME AND COMMUNITY BASED SERVICES TO OLDER ADULTS AND THE INDIGENT REGARDLESS OF FINANCIAL MEANS. WCS'S HOME AND COMMUNITY BASED SERVICES CONTINUE TO RESPOND TO STUDIES EXPRESSING A GROWING DESIRE AND NEED OF OLDER ADULTS TO RECEIVE SERVICES IN THEIR HOMES BY PROVIDING MEDICARE, MEDICAID, AND PRIVATE PAY HOME CARE SERVICES; MEALS ON WHEELS (MOW); PERSONAL AND ENVIRONMENTAL SERVICES; TRANSPORTATION SERVICES; AND OTHER VOLUNTEER SERVICES, WHICH HELP KEEP INDIVIDUALS INDEPENDENT AND ACTIVE IN THEIR HOMES AND THE COMMUNITY. WCS ALSO OPERATES THREE ADULT DAY CENTERS - WILLOWBROOK ADULT DAY SERVICES IN NEWTON, DAHL ADULT DAY SERVICES IN DES MOINES, AND LENDING HANDS ADULT DAY SERVICES IN WASHINGTON. FOR THOSE COMMUNITY MEMBERS APPROACHING END OF LIFE AND NEEDING END OF LIFE CARE, WCS ALSO PROVIDES HOSPICE, WHICH ALLOWS US TO EXTEND OUR MINISTRY AND QUALITY OF CARE. THESE SERVICES ARE CRITICAL AND FUNDAMENTAL TO ASSIST OLDER ADULTS IN REMAINING INDEPENDENT AND EMOTIONALLY CONTENT IN THEIR OWN HOMES.AS PART OF OUR MISSION, WE BELIEVE IT IS OUR RESPONSIBILITY TO MAKE THE CITIES AND COMMUNITIES IN WHICH WE OPERATE A BETTER PLACE TO LIVE. WE ARE GUIDED BY OUR CORE VALUES TO PROVIDE QUALITY SERVICES, DEMONSTRATE INTEGRITY, MAINTAIN DIGNITY, FOSTER COMMUNITY, AND DELIVER RESPONSIBLE STEWARDSHIP. WE SERVE OLDER IOWANS BY PROVIDING THEM THE QUALITY SERVICES THEY NEED WITH A SERVANT'S HEART. WE ARE COMMITTED TO QUALITY, EXCELLENCE, HOSPITALITY AND INNOVATION. WE FIRMLY BELIEVE THAT IT IS OUR RESPONSIBILITY TO BE GOOD STEWARDS OF THIS MISSION AND ONLY USE THE RESOURCES WE HAVE BEEN PROVIDED IN WAYS THAT WILL BETTER THE LIVES OF OUR CUSTOMERS. AS A NON-PROFIT ORGANIZATION, WE UNDERSTAND THAT OUR RESPONSIBILITY AND OBLIGATION TO OUR COMMUNITIES IS TO OPERATE WITH TOTAL TRANSPARENCY, HONESTY, INTEGRITY, IN PROVIDING THE BEST CARE POSSIBLE.BELOW REFLECTS SOME OF OUR COMMUNITY BENEFITS FROM 2019:- IN 2019, WE SERVED 317 PEOPLE THROUGH MEDICAID ELDERLY WAIVER AND OVER 39,060 ELDERLY WAIVER MEALS.-82.5% OF MEALS ON WHEELS CLIENTS IN 2019 RECEIVED FINANCIAL ASSISTANCE FOR THEIR MEALS. NO ONE IS TURNED AWAY FROM THIS PROGRAM BECAUSE OF THEIR INABILITY TO PAY.- OUR COMMUNITY-BASED SERVICES IMPROVE THE QUALITY OF LIFE FOR THE MANY OLDER ADULTS WHO DEPEND ON US TO MEET SOME OF THEIR MOST BASIC HUMAN NEEDS. THESE PROGRAMS ENABLE MANY TO STAY IN THEIR OWN HOMES FOR MANY YEARS, DELAYING OR ELIMINATING THE NEED FOR INSTITUTIONALIZATION. THE MOST COMMON SERVICES INCLUDED:1. TRANSPORTATION - VOLUNTEERS HELP WITH GROCERY SHOPPING AND MEAL DELIVERY FOR THOSE WHO ARE CONFINED TO THEIR HOMES. VOLUNTEERS ALSO ASSIST WITH ERRAND AND RIDES TO MEDICAL OR OTHER ESSENTIAL APPOINTMENTS. IN ADDITION TO VOLUNTEER SERVICES, WESLEY WHEELS PROVIDED ACCESS TO TRANSPORTATION TO 2,076 PEOPLE.2. VISITOR - VOLUNTEERS CALL OLDER ADULTS AND DISABLED CLIENTS REGULARLY TO ENCOURAGE SOCIALIZATION AND COMPANIONSHIP.3. MONEY MANAGEMENT - TRAINED VOLUNTEERS ARE MATCHED WITH FINANCIALLY VULNERABLE OLDER ADULTS TO HELP THEM MAINTAIN THEIR INDEPENDENCE, BE FREE OF FINANCIAL EXPLOITATION, AND INCREASE THEIR FINANCIAL LITERACY.- CLIENTS AND PARTICIPANTS OF HOME AND COMMUNITY BASED SERVICES RECEIVED $233,119 OF CHARITABLE CARE AS SUPPORTED THROUGH SCHOLARSHIPS TO HELP SUPPORT THEIR INDEPENDENCE, HEALTH AND WELL-BEING AT HOME.- THE WCS HOSPICE PROGRAM ALSO HAD A VOLUNTEER BASE OF 33 TRAINED VOLUNTEERS WHO CONTRIBUTED 975 HOURS OF ASSISTANCE TO HOSPICE PATIENTS AND THEIR FAMILIES.
