THE HEALTHCORE CLINIC, INC. (HCC) IS A FEDERALLY QUALIFIED HEALTH CENTER IN WICHITA, KS. HCC PROVIDES MEDICAL SERVICES AND OUTREACH EDUCATIONAL SERVICES, FOCUSING ON THE PATIENT'S AWARENESS OF PREVENTATIVE HEALTH CARE MEASURES. HCC SERVICES RESIDENTS IN NORTHEAST WICHITA AND SURROUNDING AREAS.
HEALTHCORE CLINIC PROVIDES ACCESS TO COMPREHENSIVE FAMILY MEDICAL SERVICES WITH A FOCUS ON EDUCATION, PREVENTION, AND EARLY INTERVENTION. THE INTEGRATED MODEL OF CARE INCORPORATES DENTAL, BEHAVIORAL HEALTH, AND SOCIAL DETERMINANTS OF HEALTH SCREENINGS AND BRIEF INTERVENTIONS THAT ARE COORDINATED DURING THE MEDICAL EXAM PROCESS. THIS INTEGRATED MODEL REDUCES STIGMA, IMPROVES ACCESS TO BEHAVIORAL HEALTH AND DENTAL SERVICES, AND PROMOTES TEAM-BASED PATIENT-CENTERED HEALTH OUTCOMES. SPECIFIC SERVICES INCLUDE: WOMEN'S HEALTH AND WELLNESS (OB/GYN CARE, FAMILY PLANNING, EARLY DETECTION WORKS (EDW) SERVICES THAT PROVIDES FREE BREAST AND CERVICAL CANCER SCREENINGS, ETC.); CHILDREN'S HEALTH AND WELLNESS (KANBEHEALTHY EXAMS, SICK AND WELL CHILD VISITS, IMMUNIZATIONS, SCHOOL AND SPORTS PHYSICALS, ETC.); AND CHRONIC DISEASE MANAGEMENT (ASTHMA, DIABETES, HYPERTENSION, HEART DISEASE, HEPC MANAGEMENT, MEDICATION MANAGEMENT, ETC.).
HEALTHCORE CLINIC PROVIDES CULTURALLY APPROPRIATE MENTAL HEALTH AND SUBSTANCE ABUSE SERVICES DESIGNED TO REDUCE STIGMA ASSOCIATED WITH SEEKING BEHAVIORAL HEALTH SUPPORT. MOST PATIENTS ARE SCREENED FOR BEHAVIORAL HEALTH CONCERNS DURING THE MEDICAL APPOINTMENT. WORKING COLLABORATIVELY WITH THE MEDICAL AND DENTAL TEAM, BEHAVIORAL HEALTH CONSULTANTS PROVIDE COORDINATED AND INTEGRATED CARE TO PATIENTS AND FAMILIES WITH CONCERNS RANGING FROM STRESS MANAGEMENT, ANXIETY, DEPRESSION, TRAUMA, AND BEHAVIORAL SUPPORT FOR OTHER MENTAL HEALTH AND MEDICAL DIAGNOSES. MENTAL HEALTH SERVICES INCLUDE: CLINICAL ASSESSMENTS; BRIEF INTERVENTIONS; PSYCHOSOCIAL SCREENINGS AND THERAPY; MEDICATION MANAGEMENT; PSYCHIATRIC EVALUATIONS; INDIVIDUAL, FAMILY, AND GROUP COUNSELING; AND REFERRAL AND COORDINATION OF SPECIALTY SERVICES WITH OTHER AGENCIES. SUBSTANCE ABUSE SERVICES INCLUDE: INITIAL CONSULTATIONS AND SCREENINGS; DRUG AND ALCOHOL EVALUATIONS; OUTPATIENT GROUP & INDIVIDUAL SESSIONS; AFTER TREATMENT CARE; CASE MANAGEMENT; ALCOHOL AND DRUG EDUCATION PROGRAM (ADEP); MEDICATION ASSISTED TREATMENT (MAT); RECOVERY/SUPPORT GROUPS (AA, NAMI, ETC.); AND OTHER EVIDENCE-BASED INTERVENTION GROUPS SUCH AS ANGER MANAGEMENT AND STRENGTHENING FAMILIES PROGRAM (SFP).
THE DENTAL CLINIC HAS SEVEN (7) CHAIRS AND DIAGNOSTIC EQUIPMENT TO PROVIDE COMPREHENSIVE DENTAL SERVICES INCLUDING EXAMS, CLEANINGS, SEALANTS, ORAL CANCER SCREENINGS, TEETH WHITENING, ROOT CANALS, DENTURES, EXTRACTIONS, CROWNS, AND WALK-IN EMERGENCY SERVICES. MOST MEDICAL PATIENTS ARE SCREENED AND PROVIDED ORAL HEALTH EDUCATION DURING ROUTINE MEDICAL APPOINTMENTS. PRENATAL, DIABETIC, AND PEDIATRIC PATIENTS RECEIVE ROUTINE DENTAL SCREENINGS AND REFERRAL TO THE DENTAL CLINIC AS NEEDED DURING MEDICAL APPOINTMENTS. THIS PROCESS HAS SUPPORTED IMPROVING ACCESS TO DENTAL SERVICES FOR TARGETED HIGH RISK POPULATIONS. FLUORIDE TREATMENTS ARE PROVIDED ALONG WITH ORAL HEALTH EDUCATION, REFERRAL FOR SERVICES AND THE DISTRIBUTION OF TOOTHBRUSHES, TOOTHPASTE AND FLOSS TO PATIENTS DURING AN INTEGRATED MEDICAL/DENTAL/BEHAVIORAL HEALTH VISIT. PATIENTS ARE ASSESSED FOR DENTAL RISK LEVEL AND EDUCATED ACCORDING TO MEDICAL DIAGNOSIS. FOR INSTANCE, DIABETIC PATIENTS RECEIVE ORAL HEALTH INFORMATION SPECIFIC TO DIABETES AND THEIR DENTAL INFORMATION IS INCLUDED WITHIN THE PATIENT'S INTEGRATED TREATMENT/CARE PLAN THAT IS REVIEWED BY ALL PROVIDERS ON THE CARE TEAM. ALSO, THE DENTAL CLINIC PROVIDES OUTREACH ACTIVITIES AT LOCAL SCHOOLS AND OTHER COMMUNITY SITES.
