Organizations Filed Purposes:
Health Equity Partners Properties (HEPP) was created in 2018 as a not-for-profit, charitable organization that is a wholly- owned subsidiary of the YMCA. The purpose of HEPP is to bring multiple collaborative services under one roof, in effort to provide community-integrated health to residents who would not otherwise have adequate access to services. HEPP holds the title to a parcel of real property located at 1720 W. Broadway, Louisville KY 40210. This facility was under construction during 2019, with official building dedication and grand opening on December 18th.
Collaborative partners have been selected to build a broad continuum of health at a single location. These include the YMCA, Norton Health Care (pediatric and family medicine clinic), ProRehab (physical therapy), Family and Children's Place (behavioral health), and Republic Bank (fiscal literacy, asset building and protection). These services will be built around combining strong and trusted brands, seamless integration of services, ease of access, strong relationships with constituents and providers, and delivery by servant leaders. Other health-related partners are expected to be drawn toward the project to further services. This may include, but not be limited to education/literacy, oral health, and others. Internally, partners will agree to collaborate in such a way that multiple needs can be met driven by building relationships and trust, and cross referencing services in a familiar, welcoming way. Barriers will be reduced and impact can be increased. This is a bold and unique project incorporating a wide array of services delivered in a highly coordinated manner.
