CDNS MISSION IS SINGULARLY FOCUSED ON PROVIDING YOUNG ADULTS WITH T1D THE PEER CONNECTIONS THEY VALUE, AND EXPERT RESOURCES THEY NEED, TO SUCCESSFULLY MANAGE THE CHALLENGING TRANSITION TO INDEPENDENCE AT COLLEGE AND BEYOND.
THE YOUNG ADULT POPULATION WITH DIABETES IS AN UNDERSERVED POPULATION, AND YET THEY ARE THE NEXT GENERATION OF DIABETES PATIENTS, DOCTORS, RESEARCHERS, AND ADVOCATES. THE COLLEGE DIABETES NETWORK (CDN) IS AN INNOVATIVE ORGANIZATION WHICH SUPPORTS THESE YOUNG ADULTS THROUGH EVERY STAGE OF THEIR TRANSITION TO ADULTHOOD, FOCUSING EXCLUSIVELY ON MEETING THEIR UNIQUE NEEDS. WE CANNOT EXPECT COLLEGE STUDENTS TO JUMP DIRECTLY INTO PERFECT MANAGEMENT PRACTICES WHILE NAVIGATING THE FIRST FEW YEARS OF COLLEGE AND INDEPENDENCE, BUT WE CAN GIVE THEM THE TOOLS TO MAKE IT EASIER. THIS STAGE OF INDEPENDENCE IS THE GOAL FOR ALL FAMILIES OF CHILDREN WITH DIABETES, AS WELL AS THE MOST INFLUENTIAL PHASE CONTRIBUTING TO SUCCESS IN ADULTHOOD. CDN CONNECTS THESE LIFE STAGES AND PROVIDES CONTINUITY OF SUPPORT WHICH REMOVES THE FEAR AND ISOLATION WHICH PREVIOUSLY DEFINED THIS TIME IN LIFE. PRIOR TO CDN THERE WAS NO PLACE FOR THESE INDIVIDUALS TO TURN, NOBODY TO KEEP THEIR NEEDS FROM BEING OVERLOOKED. CDN REPRESENTS THIS POPULATION, BRINGING THEIR VOICE TO THE REST OF THE DIABETES COMMUNITY, AND BUILDING SYSTEMS OF SUPPORT WHERE THEY ARE NEEDED. THEY CARRY OUT THEIR MISSION THROUGH THREE MAIN PILLARS OF PROGRAMMING: PROVIDING TOOLS THROUGH INFORMATION AND RESOURCES SPECIFIC TO THE NEEDS OF YOUNG ADULTS; CREATING A NETWORK OF PEERS TO EMPOWER, MOTIVATE, AND SUPPORT THEM; AND BUILDING AN ECOSYSTEM OF SUPPORT AROUND THEM. CDN'S TOOLS PROVIDE INFORMATION AND RESOURCES RELEVANT TO LIFE AS A YOUNG ADULT WITH DIABETES, AT STUDENTS FINGERTIPS, TO HELP THEM BE BETTER INFORMED AND EMPOWERED IN THEIR DIABETES MANAGEMENT. CDN'S NETWORK INCLUDES CHAPTERS ON CAMPUSES ACROSS THE COUNTRY WHICH PROVIDE PEER SUPPORT AND A LOCAL SAFETY NET OF FRIENDS WHO UNDERSTAND THE DISEASE; A NATIONAL NETWORK OF STUDENT LEADERS WHO RUN THESE CHAPTERS; AND A NATIONAL COMMUNITY OF YOUNG ADULTS LIVING WITH DIABETES. CDN'S ECOSYSTEM BUILDS BRIDGES AND CREATES COLLABORATIONS ACROSS STAKEHOLDERS WHO SUPPORT YOUNG ADULTS WITH DIABETES, FROM CLINICIANS, TO CAMPUS ADMINISTRATORS, TO PARENTS; CDN SEEKS TO CREATE SOLUTIONS WHICH ULTIMATELY IMPROVE THE OVERALL EXPERIENCE OF YOUNG ADULTS WITH DIABETES, TO HELP THEM STAY SAFER AND BE MORE SUCCESSFUL IN THEIR MANY ENDEAVORS.
