TO ASSIST CENTERS TO UNDERSTAND SERVICE NEEDS OF THEIR COMMUNITIES AND TO SUPPORT POPULATION HEALTH STRATEGIES AND SYSTEMS. TO SUPPORT CENTERS TO UNDERSTAND FEDERAL AND STATE REGULATORY REQUIREMENTS AND VALUE BASED PAYMENT SYSTEMS REQUIREMENTS FOR PROVIDING SERVICES. SUPPORT THE TECHNOLOGY SYSTEMS, EMR AND REPORTING SYSTEMS AND USE OF DATA FOR PROVIDING QUALITY, ACCOUNTABLE AND EFFECTIVE PRIMARY CARE SERVICES AT COMMUNITY HEALTH CENTERS. PROVIDE TECHNICAL ASSISTANCE TO CENTERS ON BEST PRACTICES FOR DELIVERING SERVICES AND MANAGING DATA.THE LEAGUE RESPONDED TO THE COVID19 PANDEMIC BY MOBILIZING TO WORK WITH OUR MEMBER CENTERS TO DETERMINE AND ADDRESS THEIR NEEDS. THIS INCLUDES FACILITATING COVID TESTING, STANDING UP TELEHEALTH MODALITIES, CONVENING HEALTH CENTER STAFF ON THE SWIFTLY CHANGING NEEDS SUCH AS LOSS OF REVENUE, CHANGING REGULATORY OBSTACLES, ADDRESSING WORKFORCE SHORTAGES, ESTABLISHING POP-UP SITES, CREATING PARTNERSHIPS, FACILITATING VACCINE DISTRIBUTION, ADDRESSING VACCINE HESITANCY AND WORKING WITH STATE, LOCAL AND FEDERAL PARTNERS TO ENABLE HEALTH CENTERS TO SWIFTLY AND EQUITABLY DISTRIBUTE THE VACCINES. WE ALSO ESTABLISHED A VACCINE RELIEF FUND WHICH CONSISTS OF CONTRIBUTIONS FROM PRIVATE FUNDERS TO PROVIDE DIRECT GRANTS TO HEALTH CENTER MEMBERS FOR IMMEDIATE NEEDS TO INCREASE VACCINE DISTRIBUTION.
POLICY ANALYSIS AND INFORMATION DISSEMINATION - PROVIDE SUPPORT TO COMMUNITY HEALTH CENTERS IN UNDERSTANDING FEDERAL, STATE AND LOCAL POLICIES GUIDING DELIVERY OF ACCESSIBLE AND EQUITABLE COMMUNITY BASED INTEGRATED PRIMARY CARE SYSTEMS. SUPPORT PUBLIC POLICIES THAT ENHANCE EQUITY OF CARE AND POPULATION HEALTH IMPROVEMENTS FOR VULNERABLE SAFETY-NET POPULATIONS AND COMMUNITIES. SUPPORT PUBLIC POLICY AND STRATEGIES THAT SUPPORT EFFECTIVE PAYMENT RATES AND FINANCIAL SUSTAINABILITY OF EFFECTIVE AND EQUITABLE PRIMARY CARE SERVICES AND OUTCOMES. SUPPORT CENTERS, SAFETY NET POPULATION AND COMMUNITIES TO RECEIVE INFORMATION ABOUT ACCESS TO CARE, SYSTEMS CHANGES, AND DEVELOPMENT OF POLICIES IMPACTING CARE DELIVERY SYSTEMS AND ACCESS TO EQUITABLE CARE. SUPPORT INFORMATION AND NEEDS ANALYSIS THAT ADDRESSES RESOURCE NEEDS TO SUPPORT ACCESS TO CARE AND ENGAGEMENT OF PATIENTS IN APPROPRIATE CARE AND SERVICES. WORK WITH CENTERS AND GOVERNMENT POLICY MAKERS TO DOCUMENT, ANALYZE AND ADDRESS PUBLIC HEALTH PROBLEMS AND SERVICE NEEDS.
CLINICAL HEALTH AFFAIRS AND SERVICES- PROVIDE TECHNICAL ASSISTANCE, TRAINING AND DEVELOPMENT GUIDANCE FOR THE DELIVERY OF HEALTH CARE SERVICES AND INTEGRATED COMMUNITY BASED CARE SYSTEMS. PROVIDE TRAINING AND DEVELOPMENT IN BEST PRACTICES RELATED TO QUALITY IMPROVEMENT BEST PRACTICES AND OUTCOMES MANAGEMENT. SUPPORT WORK FORCE PROGRAMS FOR DEVELOPMENT, RECRUITMENT AND RETENTION OF CLINICAL STAFF AND CLINICAL TEAM AND CARE COORDINATION AND WORKFLOW SYSTEMS FOR BEST PRACTICES. PROVIDE GUIDANCE AND TRAINING ON CLINICAL POLICIES AND BEST PRACTICES FOR DELIVERY OF PATIENT CENTERED MEDICAL HOMES AND INTEGRATED COMMUNITY BASED AND VALUE BASED PRIMARY CARE SERVICES. EXPAND COMMUNITY BASED CARE TO ADDRESS POPULATION HEALTH NEEDS AND SUPPORT DEVELOPMENT OF FOCUSED SERVICES ADDRESSING SUBSTANCE USE AND ADDICTION SERVICES.
