Organizations Filed Purposes:
TO IMPROVE THE HEALTH STATUS OF SEASONAL WORKERS AND THEIR FAMILIES BY PROVIDING CULTURALLY APPROPRIATE CARE AND SERVICES SINCE 1991, THE MAINE MOBILE HEALTH PROGRAM (MMHP) HAS CHAMPIONED THE HEALTH CARE NEEDS OF MIGRANT AND SEASONAL FARMWORKERS (MSFWS) AND THEIR FAMILIES. THE ORGANIZATION, A 501(C)(3) NONPROFIT, PROVIDES QUALITY PRIMARY, PREVENTIVE, AND SPECIALTY HEALTH SERVICES TO FARMWORKERS ACROSS THE STATE OF MAINE. FARMWORKERS ARE ONE OF THE NATION'S MOST VULNERABLE AND INVISIBLE POPULATIONS, AND THEY ARE THE ENGINE OF MAINE'S AGRICULTURAL INDUSTRY. THE HARVESTS IN MAINE EMPLOYING MIGRANT WORKERS INCLUDE BROCCOLI, POTATOES, APPLES, VEGETABLES, CHRISTMAS TREES/WREATHS, AND BLUEBERRIES. AS A VOUCHER PROGRAM, MMHP IS DEDICATED TO MAXIMIZING ACCESS TO HEALTH CARE FOR MAINE'S SEASONAL WORKERS BY PROVIDING MOBILE MEDICAL CARE, AND BY REFERRAL ARRANGEMENTS (I.E. VOUCHERS) WITH A BROAD NETWORK OF MEDICAL, DENTAL, VISION AND BEHAVIORAL HEALTH PROVIDERS. MMHP MAINTAINS CONTINUITY OF CARE F
TO IMPROVE THE HEALTH STATUS OF SEASONAL WORKERS AND THEIR FAMILIES BY PROVIDING CULTURALLY APPROPRIATE CARE AND SERVICES
COMMUNITY SERVICE - THE MAINE MOBILE HEALTH PROGRAM IS A FEDERALLY QUALIFIED HEALTH CENTER-A 330G MIGRANT VOUCHER REFERRAL PROGRAM. MMHP'S UNIQUE COMBINATION OF DIRECT CLINICAL SERVICES, VOUCHER REFERRAL, AND CONTINUOUS SUPPORT FROM COMMUNITY HEALTH WORKERS (CHW) REDUCES THE BARRIERS RELATED TO SOCIAL DETERMINANTS OF HEALTH FACED BY THESE WORKERS. WORKER HEALTH STATUS IS COMPROMISED BY POVERTY, LIVING AND WORKING CONDITIONS, SOCIAL ISOLATION, CARE DISRUPTIONS, AND LACK OF TRANSPORTATION AND CULTURALLY LINGUISTICALLY APPROPRIATE HEALTH SERVICES. MMHP SERVICES ARE INTEGRAL TO FACILITATING ACCESS TO CARE AND CHWS PROVIDE CASE MANAGEMENT, HEALTH EDUCATION, TRANSPORTATION, AND MEDICAL INTERPRETATION. THIS COMBINED APPROACH ENSURES ACCESS TO APPROPRIATE, TIMELY, AND COST-EFFECTIVE CARE. THE PROGRAM IS RESPONSIVE TO WORKER NEEDS, NIMBLE, AND HIGHLY FLEXIBLE. MMHP MAINTAINS CONTINUITY OF CARE FOR ESTABLISHED PATIENTS, REACHES OUT TO THOSE WITHOUT ACCESS, IDENTIFIES EMERGING NEEDS, AND ASSISTS COMMUNITY PROVIDERS IN RESPONDING TO THE CULTURAL AND LINGUISTIC NEEDS OF THIS DIVERSE WORKFORCE.
MEDICAL SERVICES - MMHP PROVIDES DIRECT MOBILE MEDICAL AND BEHAVIORAL HEALTH CARE AS WELL AS REFERRAL CARE THROUGH VOUCHER REIMBURSEMENT CONTRACTS WITH OVER 100 STATEWIDE ACCESS POINTS FOR MEDICAL, VISION AND BEHAVIORAL HEALTH SERVICES. THE SERVICES PROVIDED INCLUDE COMPREHENSIVE MOBILE PRIMARY AND PREVENTIVE AND BEHAVIORAL HEALTH SERVICES AND VOUCHER REFERRAL FOR SPECIALTY, VISION, LABORATORY, AND RADIOLOGY.
MMHP PROVIDES DIRECT MOBILE AS WELL AS REFERRAL CARE THROUGH VOUCHER REIMBURSEMENT CONTRACTS WITH OVER 100 STATEWIDE ACCESS POINTS FOR DENTAL SERVICES. THE SERVICES PROVIDED ALSO INCLUDE PHARMACY SERVICES ON AND OFF THE MOBILE UNITS.
