Organizations Filed Purposes: WABANAKI HEALTH AND WELLNESS IS AN AGENCY FOR TRIBALLY-ENROLLED NATIVE AMERICANS, BASED IN BANGOR, MAINE THAT OFFERS SERVICES INCLUDING CULTURALLY SENSITIVE CASE MANATEMENT; FREE HIV TESTING AND SCREENING; AND HEALTH PROMOTION, IN ADDITION, THE ORGANIZATION SERVES AS AN ADMINISTRATIVE FISCAL SPONSOR TO OTHER TRIBAL ORGANIZATIONS. THE ORGANIZATION IS SUPPORTED BY FEDERAL, STATE AND PHILANTHROPIC GRANTS INCLUDING THE SAMHSA TRIBAL OPIOID RESPONSE, PEER RUN RECOVERY CENTER, FRANNIE PEABODY PARTNERSHIP, MEHAF CAPACITY BUILDING, AND SEWALL HEALTH PEOPLE.

WABANAKI PUBLIC HEALTH - IN 2018, WABANAKI HEALTH AND WELLNESS BECAME THE STAFFING AND FINANCIAL SUPPORT AGENT FOR WABANAKI PUBLIC HEALTH. IN 2019, WABANAKI PUBLIC HEALTH TRANSFERRED THE FISCAL AGENCY AGREEMENT WITH THE PENOBSCCOT NATION HEALTH DEPARTMENT TO WABANAKI HEALTH AND WELLNESS FOR THE MAINE PREVENTION SERVICES GRANT AGREEMENTS. THE SUBSTANCE USE PREVENTION GRANT, ADMINISTERED BY THE UNIVERSITY OF NEW ENGLAND, TRANSFERRED IN APRIL OF 2019 AND THE TOBACCO PREVENTION AND OBESITY PREVENTION (ADMINISTERED BY MAINEHEALTH) AS WELL AS THE YOUTH ENGGAEMENT GRANT (ADMINISTERED BY OPPORTUNITY ALLIANCE) WERE TRANSFERRED IN OCTOBER 2019. THE SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM EDUCATION GRANT (SNAP-ED) BEGAN IN OCTOBER 2018. THE GOAL OF THE PROGRAMS IS TO PROVIDE PREVENTION SERVICES TO THE TRIBAL COMMUNITY THROUGH ACTIVITIES SUCH AS CULTURAL GATHERING, YOUTH COOUNCILS, RAISING AWARENESS OF PUBLIC HEALTH ISSUES, AND SUPPORTING COMMUNITY EVENTS, SUCH AS HEALTH FAIRS. FUNCTIONS WITHIN WPH ALSO INCLUDE SUICIDE PREVENTION (SAMHSA NATIVE CONNECTIONS, SAMHSA GARRETT LEE SMITH TRIBAL YOUTH SUICIDE PREVENTION), MENTAL HEALTH AND SUBSTANCE USE PREVENTION ADN TREATMENT (CDC OPIOID OVERDOSE PREVENTION PEER RUN RECOVERY CENTER, SAMHSA STRATEGIC PREVENTION FRAMEWORK - PARTNERSHIPS FOR SUCCESS, HRSA RURAL COMMUNITIES OPIOID RESPONSE PROGRAM - PLANNING), HEALTH COMMUNITIES (CDC TRIBAL PRACTICES FOR WELLNESS IN INDIAN COUNTRY) AND ORGANIZATIONAL OPERATIONAL CAPACITY BUILDING AND SUPPORT (CDC TRIBAL PUBLIC HEALTH CAPACITY BUILDING AND QUALITY IMPROVEMENT UMBRELLA AGREEMENT).

WABANAKI HEALTH AND WELLNESS - WABANAKI CONTINUES TO PARTNER WITH CORNERSTONE BEHAVIORAL HEALTH TO OFFER BEHAVIORAL HEALTH HOME/CASE MANAGEMENT SERVICES. CORNERSTONE HOLDS THE LICENSE AND PERFORMS ALL BILLING AND OTHER FUNCTIONS AS REQUIRED BY LICENSING AND CONTRACT. WABANAKI DIVISION STAFF ARE EMPLOYED TO PROVIDE "BEHAVIORAL HEALTH HOME" (bhho) TARGETED SERVICES TO BOTH CHILDREN AND ADULTS. BHHO IS A NEW WAY OF PROVIDING CASE MANAGEMENT USING A "WHOLE PERSON" APPROACH AND IS REIMBURSED UNDER MAINECARE. THE APPROACH INTEGRATES BOTH PHYSICAL AND MENTAL HEALTH SERVICES AND IS EXPANDING TO ADDRESS NEEDS IN MULTIPLE DOMAINS INCLUDING EDUCATIONAL, HOUSING, PEER RECOVERY AND TRANSPORTATION, ETC. CM'S HELP TO IDENTIFY THE SERVICES NECESSARY TO MEET THOSE NEEDS, COORDINATE AND FACILITATE ACCESS TO SERVICES AND INTEGRATE CARE. THIS MODEL OFFERS A TEAM-BASED APPROACH. IT BEGINS WITH INTAKE/ASSESSMENT, IDENTIFICATION OF NEEDS, DEVELOPING A PLAN OF CARE, REFERRALS, CARE COORDINATION/ADVOCADY, MONITORING, AND ENDS WHEN GOALS ARE MET. THE WABANAKI DIVISION TARGETS A VULNERABLE POPULATION OF NATIVE AMERICANS. IN PARTNERSHIP WITH COMMUNITY HOUSING OF MAINE, WABANAKI OFFERS SAFE, AFFORDABLE, AND SUPPORTIVE HOUSING TO ENROLLED NATIVE AMERICANS. OUR GOAL IS TO END HOMELESSNESS AND HELP PEOPLE ACHIEVE SELF-SUFFICIENCY. THIS YEAR WE HAVE HAD A VERY HARD TIME AND HAVE LOST CONSIDERABLE REVENUE DUE TO PARTICIPANTS WHO HAVE NOT PAID RENT AS WELL AS A SIGNIFICANT DECREASE IN SOCIAL SERVICE RESOURCES.

FISCAL SPONSORSHIPS - WABANAKI HEALTH AND WELLNESS HAS A LONG HISTORY OF PROVIDING FISCAL SPONSORSHIP SUPPORT TO OTHER TRIBAL ORGANIZATIONS NEEDING FINANCIAL, HR, OR OPERATIONAL SUPPORT. MAINE-WABANAKI REACH ENDED THEIR FISCAL SPONSORSHIP AGREEMENT WITH WABANAKI HEALTH AND WELLNESS IN 2018, FINAL PROGRAM CLOSEOUT AND ACCOUNTING COMPLETED IN EARLY JANUARY 2019. WABANAKI YOUTH AND SCINECES (WAYS) ENDED THEIR FISCAL SPONSORSHIP IN 2018; HOWEVER, STILL UTILIZES A PARTNERSHIP FOR HR AND PAYROLL SUPPORT. OTHER PARTNERSHIPS INCLUDE THE LACROSSE AND ANCESTORS PADDLE EFFORTS.
