Organizations Filed Purposes:
THE MISSION OF GRACE HOUSE IS TO SERVE HOMELESS MISSISSIPPIANS LIVING WITH AND AFFECTED BY HIV/AIDS, BY PROVIDING AND MAINTAINING HOUSING IN WHICH CLIENTS MAY LIVE WITH DIGNITY, SAFETY, AND CARE; BY PROVIDING SUPPORTIVE SERVICES THAT MEET THE PHYSICAL HEALTH, MENTAL HEALTH, EDUCATIONAL, EMPLOYMENT, AND SPIRITUAL NEEDS OF INDIVIDUAL CLIENTS THEREBY EMPOWERING EACH TO ACHIEVE THE HIGHEST QUALITY AND MOST INDEPENDENT LIFE POSSIBLE; AND BY PROVIDING EDUCATION AND AWARENESS TO THE PUBLIC.
TO PROVIDE HOUSING AND SUPPORTIVE SERVICES FOR HIV/AIDS+ PEOPLE AND TO PROVIDE EDUCATION TO THE PUBLIC.
HOPWA SERVICES WHICH INCLUDE ALL GRANTS ON THE SEFA EXCEPT THOSE TITLED CONTINUUM OF CARE HOMELESS ASSISTANCE THESE PROGRAMS PROVIDE PERMANENT HOUSING SOLUTIONS AND CASE MANAGEMENT TO HOUSEHOLDS WHO ARE HOMELESS OR AT RISK OF BECOMING HOMELESS AND HAVE AT LEAST ONE QUALIFYING HOUSEHOLD MEMBER LIVING WITH HIV. THE PROJECT ASSISTS APPROXIMATELY 500 PERSONS ANNUALLY WITH HOUSING, CASE MANAGEMENT, AND OTHER SUPPORTIVE SERVICES. 1) ENSURED THAT 60 HOUSEHOLDS ASSISTED THROUGH ANOTHER AREA PROGRAM THAT CLOSED DID NOT BECOME HOMELESS. 2) ASSISTED CONSISTENTLY THROUGHOUT THE YEAR 300 350 HOUSEHOLDS ENSURING THAT THEY DID NOT BECOME OR REMAIN HOMELESS.3) SUCCESSFULLY MOVED 89.96% OF ALL HOUSEHOLDS ASSISTED IN ONGOING PERMANENT HOUSING. 4) HELPED THOSE MOST IN NEED WITH 85% OF ALL ASSISTED HOUSEHOLDS BEING AT OR BELOW 30% OF AREA MEDIAN INCOME (AMI) AND 95% BELOW 50% AMI. THE REMAINING 5% OF ASSISTED HOUSEHOLDS WERE BELOW 80% AMI. 5) LASTLY, 47.7% OF ALL HOUSEHOLDS ASSISTED HAD AN INCREASE IN HOUSEHOLD INCOME THROUGHOUT THE PROGRAM YEAR.
CONTINUUM OF CARE HOMELESS ASSISTANCE GRANTS OUTCOMES:THE PROGRAM PROVIDES PERMANENT HOUSING TO FAMILIES AND INDIVIDUALS WHO ARE HOMELESS AND HAVE AT LEAST ONE DISABLING CONDITION INCLUDING HIV, CHEMICAL DEPENDENCY, MENTAL ILLNESS, OR OTHER PHYSICAL CONDITIONS. THE PROJECT ASSISTS OVER 100 HOUSEHOLDS ANNUALLY WITH HOUSING, CASE MANAGEMENT, AND OTHER SUPPORTIVE SERVICES. 1) SERVED 117 PREVIOUSLY HOMELESS HOUSEHOLDS ENSURING THAT 95% REMAINED HOUSED WITHOUT RETURNING TO HOMELESSNESS.2) CASE MANAGERS PROVIDED SUPPORTIVE SERVICES THAT LEAD TO 42% OF ALL HOUSEHOLDS ASSISTED IN INCREASING THEIR INCOME DURING THE YEAR.3) ASSIST CONSISTENTLY THROUGHOUT THE YEAR 43 HOUSEHOLDS ENSURING THEY DID NOT RETURN TO HOMELESSNESS.
