Children and adults with disabilities and special needs find highest-quality services designed to meet their individual needs when they come to Easter Seals. Teams of therapists, case managers, care coordinators and other health professionals help each person overcome obstacles to independence and reach his or her personal goals. Easter Seals also includes families as active members of any therapy program, and offers the support families need.Military & Veterans Services:Our mission is to provide critical and timely financial assistance when no other resource is available to veterans, service members and their families, to ensure their dignity, health and overall well-being. The crisis facing the nation in meeting the physical and mental health needs of the Armed Forces having served in Iraq and Afghanistan is overwhelming and continues to grow. Thousands of injured military service members are returning home with hopes of successfully transitioning to civilian life. Many communities are not equipped to respond appropriately to their unique need, nor are they aware of how to best work with military and Veterans systems in the process. Support for these services is funded by Veterans Count Maine's fundraising board and philanthropic efforts. Easterseals Maine's Military & Veterans services are working hard to fill these gaps in services. We offer military and Veterans systems of care with viable options to support and augment current reintegration efforts. Clinical Services:Easterseals Outpatient Counseling provides mental health counseling to children and families in a warm, clinical setting conveniently located in Portland. Treatment is a strength-based, person-centered approach that supports the development of skills required to manage mental health symptoms so that they may grow, thrive, and experience success in all areas of their lives. Counseling can address a range of mental health diagnoses, including but not limited to Post-Traumatic Stress Disorder, Anxiety Disorders, Autism and Oppositional-Defiance Disorders. Our staff are Licensed Clinicians who offer a range of treatment models including Trauma-Focused Cognitive Behavioral Therapy, social skills training, play therapy and family therapy.Targeted Case Management Services:Targeted Case Management (TCM) is a strengths based, child focused service that supports children and their families in identifying and accessing supports and resources required to effectively manage children's mental and physical health needs. TCM supports children and their families in at least twice per month meetings where they will advocate for a child's needs across settings, make appropriate referrals to community resources, and work with the family and other providers to strengthen the treatment team. Family needs and family decisions guide our Case Managers in the effort to help each child and family obtain individualized services and experience success. To access TCM services, children must access MaineCare as well as have a mental or physical health diagnosis, which could include but are not limited to Post-Traumatic Stress Disorder, ADHD and Autism Spectrum Disorder. Services available are assessment of strengths and needs, creation of individualized treatment plan, identifying and accessing appropriate services and resources, family advocacy and care coordination, advocate for educational needs and attend school meetings to ensure child's educational needs are met and addressed and ongoing support and monitoring of progress.
