Healthy Here: Communities Leading Healthy Change - BCCHC serves as the guiding coalition for this collective impact initiative involving multiple community partners who are working to increase access to healthy food, opportunities for physical activity, and clinical/ community linkages with the goal of reducing and preventing chronic disease for Hispanic/Latinos/Chicanos and Native Americans living in the International District, South Valley, and parts of the North Valley.
