Organizations Filed Purposes:
TCHC leads the community solution to homelessness by serving as a catalyst for community change. TCHC is the lead agency and collaborative applicant for the Fort Worth/Arlington/Tarrant County Continuum of Care.
Coordinated Entry is a system-wide, "no wrong door" system, designed to ensure anyone experiencing homelessness has fair and equal access to the community's housing resources. Through this process and system, households are quickly identified, assessed, and rapidly served with the most appropriate housing solution.
CONTINUUM OF CARE PLANNING - THE CONTINUUM OF CARE BOARD OF DIRECTORS, BY RESOLUTION, APPOINTED TCHC AS THE COLLABORATIVE APPLICANT AND LEAD AGENCY TO CARRY OUT CONTINUUM OF CARE OPERATIONS, AS REQUIRED BY HUD. THE CONTINUUM OF CARE PROGRAM GRANT AUTHORED AND SUBMITTED BY TCHC ON BEHALF OF THE COMMUNITY IS SUBJECT TO NATIONAL COMPETITION. THE GRANT IS AN EXHAUSTIVE DOCUMENT THAT GIVES DETAILS ON HOW THE COMMUNITY OPERATES, MONITORS, EVALUATES, IMPROVES AND DEVELOPS ITS EMERGENCY AND STABILIZATION SYSTEMS AND PROGRAMS FOR PERSONS HOW ARE AT RISK OF OR EXPERIENCING HOMELESSNESS. CONTINUUM OF CARE OPERATIONS ALSO INCLUDES THE NEW COORDINATED ASSESSMENT SYSTEM WHICH PROVIDES A HOMELESS HELPLINE FROM 8 AM DAILY TO PROVIDE INFORMATION AND REFERRAL AND DIVERT PERSONS WITH A HOUSING CRISIS FROM ENTERING IN TO THE HOMELESS SYSTEM. THE SYSTEM ALSO PROVIDES A MECHANISM FOR IDENTIFYING, DOCUMENTING AND PRIORITIZING PERSONS SEEKING ASSISTANCE TO ASSURE THAT PROGRAMS DESIGNED TO SERVE THE CHRONICALLY HOMELESS, VETERANS, FAMILIES WITH CHILDREN, OR PRESENTING WITH OTHER DISABILITIES ARE TARGETED AND SERVING THOSE WITH THE HIGHEST LEVEL OF NEED. THE SYSTEM ALSO MAINTAINS AN INVENTORY OF HOUSING AND INTERVENTION RESOURCES TO ASSURE THAT PROGRAMS ARE AT THEIR HIGHEST OCCUPANCY AND MAINTAIN THE MOST EFFICIENT USE OF AVAILABLE PUBLIC RESOURCES.
Client Services through Partner Agencies A community fund, which is a flexible fund of last resort to provide rapid assistance for items that are not covered by other housing grants. Assistance provided includes: homeless diversion and housing and household expenses such as deposits, application fees, high risk fees, administrative fees, mattresses, welcome home packages and motel stays for cold weather.
