THE WELLNESS CENTER IS LOCATED IN DALLAS COUNTY AND INCORPORATED IN THE STATE OF TEXAS. OUR MISSION IS TO PROVIDE HEALTH CARE AND HEALTH-RELATED EDUCATION TO LOW INCOME FAMILIES WHO ARE UNABLE TO ACCESS MAINSTREAM HEALTH CARE. IN 2013 WE PROVIDED SERVICES TO 2,477 PATIENTS. IN 2013, OUR MEDICAL SERVICES, WHICH INCLUDE PRIMARY CARE PHYSICIAN SERVICES, DIETETIC SERVICES, EYE EXAMS, CHILD/FAMILY COUNSELING, IMMUNIZATIONS, LAB WORK, PHARMACY ASSISTANCE, PODIATRY CARE AND WOMEN'S HEALTH. AT RETAIL, AND INCLUDING THE VALUE OF IN-KIND SERVICES DONATED TO THE ORGANIZATION SERVICES PROVIDED TOTALED $1,195,365 OR $482.59 PER PATIENT.

A SECOND HEALTH CARE CATEGORY PROVIDED BY THE ORGANIZATION IS DENTAL CARE, WHICH INCLUDES DENTAL AND DENTAL HYGIENE SERVICES. IN 2013, WE PROVIDED SERVICES TO 959 CHILDREN AND ADULT PATIENTS IN OUR FACILITY. PREVENTIVE DENTAL CARE IS PRACTICED BY EDUCATING PATIENTS AND THE COMMUNITY ON ORAL HEALTH AND ITS EFFECT ON OVERALL HEALTH, BY PROVIDING DIETARY EDUCATION, AND THROUGH COUNSELING. IN 2013, AT RETAIL AND INCLUDING THE VALUE OF IN-KIND SERVICES DONATED TO THE ORGANIZATION, DENTAL SERVICES PROVIDED TO OUR PATIENTS WOULD BE VALUED AT $157,323. PER PATIENT THE AVERAGE SERVICES PROVIDED WERE VALUED AT $164.05.

A THIRD CATEGORY OF PROVIDED SERVICES IS HEALTH SCREENINGS, WHICH ARE GIVEN AT THE FACILITY THROUGHOUT THE MONTH, AND AT A HEALTH FAIR, ALSO AT OUR FACILITY, ON THE LAST SATURDAY OF THE MONTH. ADDITIONALLY, WE SPONSOR AND/OR PARTICIPATE IN HEALTH FAIRS OFFERED AT DIFFERENT LOCATIONS THROUGHOUT THE COMMUNITY. SCREENINGS/HEALTH MANAGEMENT SERVICES INCLUDE MAMMOGRAPHY, BLOOD PRESSURE, DIABETES, CHOLESTEROL, HEARING, VISION AND BMI ANALYSIS. IN 2013, SCREENING/HEALTH MANAGEMENT SERVICES WERE PROVIDED TO 2,080 CHILDREN AND ADULTS. AT RETAIL, AND INCLUDING THE VALUE OF SERVICES DONATED TO THE ORGANIZATION, THESE SERVICES WOULD BE VALUED AT $77,202. PER PATIENT, THE AVERAGE VALUE OF SERVICES PROVIDED WAS $37.12.
