DEVELOPED A GUIDED TRANSITION PROGRAM TO ASSIST YOUTH WITH DISABILITIES & THEIR FAMILIES SUCCESSFULLY TRANSITION TO LIFE AFTER HIGH SCHOOL. GATHERED OVER 1200 RESOURCES FOR THE HEALTH & HUMAN SERVICES COMMISSION REGIONS 2 & 3 SECTIONS OF THE NAVIGATE LIFE TEXAS WEBSITE. PARTNERED WITH 50 OTHER AGENCIES TO BRING THE CARESTARTER CONNECTIONS APP TO HOUSTON - THIS FREE APP ALLOWS FAMILIES TO ACCESS RESOURCES FROM THEIR PHONES. OVER 400 FAMILY MEMBERS PARTICIPATED IN 8 EVENTS HELPING BUILD NETWORKS OF SUPPORT AMONG FAMILIES. HAVE OVER 1700 PERSONS SUBSCRIBING TO OUR MONTHLY EDIGEST OF EVENTS FOR FAMILIES,859 MEMBERS OF OUR FACEBOOK GROUP, AND ANOTHER 96 MEETUP GROUP MEMBERS. THROUGH OUR PROGRAMS, FAMILIES ARE ABLE TO NAVIGATE THE EDUCATION, HEALTH CARE, AND SOCIAL SYSTEMS AND GAIN SUPPORT FOR THIER CHILDREN. WITH SUPPORTS & SERVICES, CHILDREN WITH DISIBILITIES WILL BE ABLE TO ACHIEVE POST SECONDARY EDUCATION, EMPLOYMENT OR INDEPENDENCE. DONATED SERVICES OF $14,191.
TEXAS PROJECT FIRST: PROVIDE ACCURATE AND CONSISTENT INFORMATION ON THE SPECIAL EDUCATION PROCESS IN A BILINGUAL FORMAT. THIS PROJECT IS A COLLABORATIVE PROJECT WITH THE TEXAS EDUCATION AGENCY AND REGION 9 EDUCATION SERVICE CENTER TO FULFILL THE GOALS OF THE TEXAS CONTINUOUS IMPROVEMENT PROCESS. THE WEBSITE RECEIVED APPROXIMATELY 3 MILLION HITS ON SEPT 2014-AUGUST 2015 CONTRACT YEAR. IT HAS OVER 2,900 INDIVIDUALS WHO SUBSCRIBE TO RECEIVE UPDATES AND IMPORTANT INFORMATION DISSEMINATED BY THE TEXAS EDUCATION AGENCY.
