Organizations Filed Purposes: To improve quality and reduce costs through coordination of care and data integration for the most costly and vulnerable in our health care system.

Current models of care for vulnerable populations are siloed across and within medical and social providers, and the Organization is scaling its care coordination and technical platform to build a coordinated approach to care for vulnerable people to improve their quality of life. We work to ensure accountability across medical and social providers. These individuals share a platform that will help providers track, measure, and address their clients' complex determinants of health - including medical, housing and transportation, income and employment status, hunger and access to nutritious foods, literacy and language, behavioral health, and social integration. The last 128 clients showed a 20% improvement in the quality of life and a 37% reduction in visits to hospitals. In addition, the telehealth platform coordinated over 16,000 social or medical service sessions for over 13,000 clients. $3.5 million was saved in health & social service expenses. Last year, over 600 partner agencies were added which resulted in over 1,900 community resources. The database now incudes over 2.7 million client records after integrating variables from public data sets.
