THE MISSION OF THE COMMUNITY HEALTH CENTERS OF BURLINGTON (CHCB) IS TO IMPROVE THE HEALTH OF ALL WITHIN THE COMMUNITIES CHCB SERVES. CHCB IS DEDICATED TO PROVIDING EXCEPTIONAL CARE WITH RESPECT AND COMPASSION AND WITH A COMMITMENT TO SERVING PEOPLE REGARDLESS OF FINANCIAL STATUS OR LIFE CIRCUMSTANCE.
THE COMMUNITY HEALTH CENTERS OF BURLINGTON (CHCB) IS A PRIMARY CARE FAMILY PRACTICE AND A FEDERALLY QUALIFIED HEALTH CENTER OFFERING MEDICAL, DENTAL, LABORATORY, PSYCHIATRY, COUNSELING, SUBSTANCE ABUSE AND CASE MANAGEMENT SERVICES AS PART OF AN INTEGRATED HEALTH CARE HOME. CHCB SERVES ALL MEMBERS OF THE COMMUNITY REGARDLESS OF LIFE CIRCUMSTANCE, INCOME OR INSURANCE STATUS. CHCB OFFERS A SLIDING-FEE SCALE FINANCIAL ASSISTANCE PROGRAM FOR ALL SERVICES BASED ON INCOME WITHIN THEIR DEFINED SERVICE AREA, AND AT NO COST TO THOSE QUALIFIED AS HOMELESS. CHCB'S EIGHT HEALTH CENTERS ARE LOCATED IN CHITTENDEN AND SOUTHERN GRAND ISLE COUNTIES. THE FOLLOWING ARE A FEW OF THE FISCAL YEAR 2020 HIGHLIGHTS THAT CHCB HAS ACHIEVED: -CHCB ENDED THE FISCAL YEAR PROVIDING 128,282 PATIENT VISITS. -KNOWING THAT NEW AMERICANS AND LIMITED ENGLISH COMMUNITY RESIDENTS OFTEN STRUGGLE WITH HISTORIES OF TRAUMA AND COMPLEX MEDICAL CONDITIONS, CHCB CONTINUALLY PROVIDES INTERPRETER-ASSISTED VISITS TO ENSURE CULTURALLY COMPETENT CARE. IN FY20, THE MOST SPOKEN NON-ENGLISH LANGUAGE WAS NEPALI, FOLLOWED BY SOMALI AND VIETNAMESE. -AS ONE OF THE LARGEST FAMILY PRACTICES OFFERING MEDICATION ASSISTED TREATMENT, CHCB NOW CARES FOR ALMOST 600 PATIENTS RECEIVING SUBOXONE TREATMENT AS PART OF VERMONT'S ONGOING FIGHT AGAINST OPIOID ADDICTION. -THANKS TO PHILANTHROPIC SUPPORT, CHCB RECEIVED FUNDING TO PURCHASE A NEW HIGH-RESOLUTION ULTRASOUND AND A NEW NON-STRESS TEST MACHINE FOR PRENATAL SCREENINGS. ALSO THROUGH PHILANTHROPIC SUPPORT, FACILITY IMPROVEMENTS, INCLUDING NEW BOILERS AND FLOORING, WERE MADE AT CHCB'S SAFE HARBOR HEALTH CENTER, THE PRIMARY SITE FOR HOMELESS HEALTH CARE PROGRAMMING. CHCB WAS ALSO FORTUNATE TO RECEIVE FUNDING THROUGH THE HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA) FOR ALMOST 300,000 TO INCREASE THEIR ORAL HEALTH SERVICE CAPACITY BY COMPLETING THE INFRASTRUCTURE FOR TWO ADDITIONAL DENTAL OPERATORIES AT THEIR SOUTH END HEALTH CENTER, ALONG WITH CUTTING EDGE DENTAL EQUIPMENT AND TECHNOLOGY THROUGHOUT THEIR ORGANIZATION. -IN FY19, CHCB BEGAN OFFERING TELE-PSYCHIATRY AND TELE-THERAPY APPOINTMENTS FOR HOMEBOUND INDIVIDUALS. IN FY20, BROADER TELE-HEALTH APPOINTMENTS WERE LAUNCHED AT THE PEARL STREET YOUTH HEALTH CENTER AND, WITHIN A MATTER OF MONTHS, TELE-HEALTH OFFERINGS WERE RAPIDLY EXPANDED IN RESPONSE TO THE PUBLIC HEALTH CRISIS CAUSED BY COVID-19. BY THE END OF THE FISCAL YEAR, UPWARDS OF 85% OF VISITS WERE PERFORMED THROUGH TELE-HEALTH TECHNOLOGY, REDUCING CONTAMINATION RISKS AND ENSURING PATIENT AND STAFF SAFETY.
