THE GUIDANCE CENTER DELIVERS BEHAVIORAL HEALTH SOLUTIONS.
ADULT OUTPATIENT SERVICES: SCOPE OF PRACTICE IS MEDICATION SERVICES, BEHAVIORAL HEALTH, CRISIS, PRE-PETITION SCREENING, AND COUNSELING. THE PROGRAM DEPARTMENTS INCLUDE SERVICES FOR SERIOUS MENTAL ILLNESS, ASSERTIVE COMMUNITY TREATMENT, GENERAL MENTAL HEALTH SUBSTANCE ABUSE, CRISIS AND 23-HOUR CRISIS STABILIZATION, MEDICATION CLINIC AND OUTPATIENT NURSING, AND CLINICS OFFERED IN WILLIAMS, TUBA CITY AND HAVASUPAI. THE OVERALL GOAL IS TO IMPROVE THE BEHAVIORAL HEALTH OF ADULTS WITH SERIOUS MENTAL ILLNESSES OR OTHER MENTAL HEALTH PROBLEMS THROUGH THE PROVISION OF HIGH-QUALITY, COST EFFECTIVE OUTPATIENT SERVICES. COUNSELING IS OFFERED TO CONSUMERS IN ORDER TO ASSIST WITH THE IDENTIFICATION OF PROBLEMS OR CONFLICTS, TO PROVIDE SUPPORT, EDUCATION, AND UNDERSTANDING FOR THE CONSUMER, GROUP OF CONSUMERS OR FAMILY IN ORDER TO RESOLVE OR MANAGE FUTURE PROBLEMS OR CONFLICTS LIVING SKILLS TRAINING AND CASE MANAGEMENT SERVICES ARE PROVIDED TO ASSIST THE CONSUMER IN MAXIMIZING HIS/HER ABILITY TO LIVE AND PARTICIPATE IN THE COMMUNITY AND TO FUNCTION INDEPENDENTLY BY TEACHING INDEPENDENT LIVING, SOCIAL AND COMMUNICATION SKILLS TO CONSUMERS AND FAMILIES.FY2019 ACCOMPLISHMENTS: ADULT OUTPATIENT SERVICES INCREASED GROUP THERAPY OFFERINGS TO MORE THAN 20 GROUPS OFFERED WEEKLY IN FLAGSTAFF. TOPICS RANGE FROM MATRIX SUBSTANCE USE (INTENSIVE OUTPATIENT PROGRAM, OR IOP) TO ANGER MANAGEMENT, TRAUMA & RECOVERY, BUILDING HEALTH AND WELLNESS, ANXIETY MANAGEMENT, MINDFULNESS, AND CONTINUATION OF THE BUILDING OF DIALECTICAL BEHAVIOR THERAPY (DBT) FOR WHICH TGC SERVES AS A STATEWIDE CENTER OF EXCELLENCE. THE ASSERTIVE COMMUNITY TREATMENT (ACT) TEAM REMAINS THE ONLY ACT TEAM IN NORTHERN ARIZONA, SERVING DURING THIS FISCAL YEAR 35-40 INDIVIDUALS WITH HIGH-RISK SERIOUS MENTAL ILLNESSES, OFTEN WITH CO-OCCURRING SUBSTANCE USE ISSUES. ACT'S GOALS REMAIN HELPING THOSE THEY SERVE STAY HOUSED, SAFE FROM LAW ENFORCEMENT INVOLVEMENT AND HAPPY IN HOME AND COMMUNITY-BASED RELATIONSHIPS.CHILD & FAMILY SERVICES - A WIDE CONTINUUM OF SERVICES DESIGNED TO SUPPORT A FAMILY-CENTERED SERVICE DELIVERY MODEL, OFFERING PROGRAMS TO CHILDREN FROM BIRTH TO 18 YEARS OLD. THE OVERALL GOAL IS TO PROVIDE SERVICES FOR CHILDREN AND FAMILIES IN NEED OF BEHAVIORAL HEALTH SUPPORT IN AN EASILY ACCESSIBLE MANNER AND IN A SAFE, SECURE, AND CONFIDENTIAL ENVIRONMENT. CHILDREN'S OUTPATIENT PRIMARY SERVICES PROVIDE INCLUDE: INDIVIDUAL, FAMILY AND GROUP THERAPY, CARE MANAGEMENT, SKILLS TRAINING, AND FAMILY SUPPORT.FY2019 ACCOMPLISHMENTS: CHILD & FAMILY SERVICES OVERCAME STAFFING DIFFICULTIES TO FORM A STRONG TEAM OF PASSIONATE SERVICE PROVIDERS. IN THIS FISCAL YEAR, GROUNDWORK WAS LAID TO BEGIN A PARTNERSHIP WITH MOUNTAIN VIEW PEDIATRICS FOR SMOOTHER TRANSITIONS OF CARE AND IMPROVED COORDINATION FOR FAMILIES. ADDITIONALLY, TGC BEGAN RECEIVING REFERRALS TO SERVE BIRTH-TO-FIVE MEMBERS & FAMILIES FROM NAVAJO COUNTY SINCE THAT SPECIALTY OF SERVICES IS NOT AVAILABLE IN THE AREAS SERVED BY ONE OF TGC'S SISTER AGENCIES. FINALLY, SERVICES PROVIDED WITHIN SCHOOLS INCREASED SUBSTANTIALLY OVER PREVIOUS YEARS AS IN-SCHOOL BEHAVIORAL HEALTH HAS BEEN MORE NEEDED THAN EVER FOR CRISIS INTERVENTION AND INDIVIDUAL STUDENT SUPPORT.CRISIS & INTAKE SERVICES - TIED TO THE STATEWIDE CRISIS LINE AND PARTNERED WITH ANOTHER PROVIDER FOR MOBILE CRISIS SERVICES, TGC'S CRISIS SERVICES IS STAFFED 24/7 WITH MASTER'S LEVEL CLINICIANS AND BACHELOR'S LEVEL CARE MANAGERS WHO HELP INDIVIDUALS STRUGGLING WITH COMPLEX ISSUES WITH AN ASSESSMENT AND DIRECT REFERRAL INTO THE CRISIS STABILIZATION UNIT (CSU) OR PSYCHIATRIC ACUTE CARE (PAC). CRISIS CLINICIANS ALSO PROVIDE ASSESSMENTS IN THE COCONINO COUNTY DETENTION FACILITY. CRISIS CLINICIANS EVALUATE INDIVIDUALS WHO MAY BE REFERRED FOR POSSIBLE INVOLUNTARY COMMITMENT SHOULD THEIR RISK LEVEL DEMAND IT AND THEY ARE NOT WILLING TO ACCEPT TREATMENT. FY2019 ACCOMPLISHMENTS: CRISIS SERVICES REMAINS FULLY AVAILABLE ON-SITE 24/7 AND THIS YEAR, THE INTAKE PROCESS FOR INDIVIDUALS SEEKING INITIAL SERVICES HAD A SYSTEMIC MAKEOVER. ALL CRISIS CLINICIANS WERE TRAINED TO PROVIDE INTAKES. TGC BEGAN WALK-IN INTAKES IN SUMMER 2019, AVAILABLE 7 DAYS A WEEK. THE TIME FROM INITIAL CALL OR REFERRAL TO INTAKE COMPLETION WAS REDUCED, ON AVERAGE, FROM ABOUT 20 DAYS TO LESS THAN 2 DAYS. IN RESPONSE TO FEEDBACK THAT WALK-IN ONLY OPTIONS WERE NOT WORKING FOR ALL COMMUNITY MEMBERS, APPOINTMENTS WERE AGAIN OFFERED; HOWEVER, NO APPOINTMENT IS SCHEDULED MORE THAN SEVEN DAYS FROM FIRST CONTACT AND MOST ARE NEXT-DAY APPOINTMENTS. INTAKES WERE ALSO SHORTENED BY HALF TO ABOUT 75 MINUTES TO REDUCE LIFE IMPACT ON INDIVIDUALS SEEKING SERVICES.WILLIAMS CLINIC: AN OUTPATIENT CLINIC PROVIDING INTAKES, PSYCHIATRIC OUTPATIENT SERVICES, CARE MANAGEMENT AND THERAPY SERVICES TO OVER 210 INDIVIDUALS. WHILE MOST SERVICES ARE PROVIDED IN-PERSON, ADDITIONAL SERVICES AVAILABLE IN FLAGSTAFF CAN BE CONNECTED TO THE WILLIAMS CLINIC VIA TELEMEDICINE.FY2019 ACCOMPLISHMENTS: WILLIAMS CLINIC NOW OFFERS VOCATIONAL REHABILITATION SERVICES AND GROUP THERAPY FOR ADULTS WITH SUBSTANCE USE NEEDS; ADULTS NEEDING STRUCTURED COPING SKILLS TO GET THROUGH TRAUMA AND LIFE CHALLENGES; AND, SOCIALIZATION WITH MINDFULNESS SKILL DEVELOPMENT.
THE SUBSTANCE ABUSE RESIDENTIAL RECOVERY PROGRAM: A 16-BED, CO-ED ADULT UNIT SERVING HIGH-RISK INDIVIDUALS NEEDING INTENSIVE TREATMENT WITH RESPITE FROM THEIR LIFE IN THE COMMUNITY. SERVICES ARE PRIORITIZED TO PREGNANT WOMEN, WOMEN WITH DEPENDENT CHILDREN, INTRAVENOUS DRUG USERS AND INDIVIDUALS STRUGGLING WITH OPIOIDS. FY2019 ACCOMPLISHMENTS: SARRP SERVED 167 ADULTS WITH MODERATE TO SEVERE SUBSTANCE USE DISORDERS AND OFTEN CO-OCCURRING MENTAL AND PHYSICAL HEALTH CONDITIONS AS WELL. PROGRAMMING HAS BEEN SUBSTANTIALLY MODIFIED THIS YEAR TO REFLECT MORE CURRENT, EVIDENCE-BASED SKILLS TRAINING AND PSYCHOTHERAPY. PSYCHOTHERAPY AVAILABILITY WAS ENHANCED THROUGHOUT THE YEAR. PARTNERSHIPS FOR EXPEDITED PROBATION, COURT AND DETENTION REFERRALS WERE DEVELOPED TO BETTER SERVE INDIVIDUALS ENCOUNTERING THE JUSTICE SYSTEM WHILE STRUGGLING WITH SUBSTANCE USE.
PSYCHIATRIC ACUTE CARE UNIT: A 16-BED LEVEL I ACCREDITED PSYCHIATRIC HOSPITAL SERVING HIGH-RISK INDIVIDUALS NEEDING INTENSIVE TREATMENT FOR STABILIZATION FROM DANGER AND DYSFUNCTION. SERVICES INCLUDE TITLE 36 PRE-PETITION SCREENING/ COURT-ORDERED EVALUATIONS AND TREATMENT, PSYCHIATRIC ACUTE CARE, AND RESTRAINTS/ SECLUSIONS CAPABILITY. THE FACILITY ACCOMMODATES INDIVIDUALS WITH MANY TYPES OF DISABILITIES. THE UNIT PROVIDES HOSPITALIZATION, PSYCHIATRIC EVALUATION, MEDICATION STABILIZATION, 24-HOUR NURSING, TELEHEALTH AND EMERGENCY PSYCHIATRIC CARE WHICH INCLUDES VOLUNTARY AND INVOLUNTARY HOSPITALIZATION.FY2019 ACCOMPLISHMENTS: PEER SUPPORT WAS ENHANCED AT THE HOSPITAL SO INDIVIDUALS WITH LIVED EXPERIENCE COULD HELP HOSPITALIZED INDIVIDUALS GAIN INSIGHT INTO THEIR HEALTH CONDITIONS AND NEED TO ADHERE TO PRESCRIBED MEDICATIONS. SIGNIFICANT IMPROVEMENTS IN THE ASSESSMENT AND TREATMENT OF INVOLUNTARILY COMMITTED INDIVIDUALS LAUNCHED THIS FISCAL YEAR WITH A GROWING PARTNERSHIP WITH THE COCONINO COUNTY ATTORNEY'S OFFICES.
