THE AZTLC INTEGRATED DELIVERY MODEL FOCUSES ON ELIMINATING BARRIERS TO CARE THROUGH A CLIENT-CENTERED HEALTH HOME EMPHASIZING PERSONAL HEALTH RESPONSIBILITY
THE AZTLC TARGET POPULATION IS THE UNDERSERVED RESIDENTS LIVING IN THE CENTRAL AND SOUTH PHOENIX SERVICE AREA, ESPECIALLY THOSE RESIDENTS LACKING A COMPREHENSIVE HEALTH HOME. AZTLC ALSO PROVIDES SERVICES THROUGHOUT THE SERVICE AREA AT NUMEROUS COMMUNITY-BASED LOCATIONS INCLUDING INDIVIDUAL RESIDENCES, SCHOOLS, AND NURSING HOMES THROUGHOUT METRO-PHOENIX. WITHIN THE AZTLC SERVICE AREA, NEARLY 56 PERCENT OF THE POPULATION HAS HOUSEHOLD INCOMES LESS THAN 200% OF THE FEDERAL POVERTY LEVEL, 20 PERCENT OF THE RESIDENTS ARE UNINSURED, AND LACK ACCESS TO AFFORDABLE INTEGRATED HEALTHCARE. OVER 46 PERCENT OF THE POPULATION IS LINGUISTICALLY ISOLATED. NEARLY 20 PERCENT OF THE SERVICE AREA BIRTHS ARE TO TEENAGE MOTHERS. OVER 60 PERCENT OF THE CHILDREN HAVE NOT RECEIVED IMMUNIZATIONS 4-3-1-3-3, AND THE YOUTH SUICIDE ATTEMPTS REQUIRING MEDICAL CARE IS HIGHER THAN THE NATIONAL LEVEL. OTHER MAJOR HEALTHCARE NEEDS INCLUDE THE EARLY DIAGNOSIS OF DEVELOPMENT AND EMOTIONAL CONDITIONS IN CHILDREN, CHRONIC CONDITIONS SUCH AS HYPERTENSION, DIABETES, OBESITY, AND ASTHMA IN ADULTS WITH THE PREVALENCE OF A BEHAVIORAL HEALTH CONDITION, AND POOR COPING SKILLS THROUGH OVEREATING, SMOKING, DRINKING AND SUBSTANCE ABUSE.
