THROUGH ITS AFFILIATES, FOUNDATION FOR SENIOR LIVING (FSL) HAS BEEN CARING FOR ARIZONANS SINCE 1974. WE PROVIDE A SAFE HOME FOR NEARLY 1,000 ARIZONANS EACH NIGHT THROUGH THE DEVELOPMENT OF BOTH SINGLE- AND MULTI-FAMILY HOMES AND THE MANAGEMENT OF MULTI-FAMILY AFFORDABLE HOUSING COMPLEXES AND RESIDENTIAL BEHAVIORAL HEALTH GROUP HOMES. ADDITIONALLY, WE TRAIN AND MONITOR SPONSORS OF THE ADULT FOSTER CARE AND ADULT BEHAVIORAL HEALTH THERAPEUTIC HOME PROGRAMS. IN FISCAL YEAR 2019-20, WE PROVIDED NEARLY 500,000 MEALS TO FAMILIES IN NEED VIA OUR RURAL FOOD PANTRIES, CONGREGATE AND HOME-DELIVERED MEAL PROGRAMS. (CONTINUED ON SCHEDULE O) HOME AND COMMUNITY-BASED SERVICES ARE PROVIDED TO ENHANCE INDEPENDENCE FOR PEOPLE OF ALL AGES, INCLUDING ADULT DAY HEALTH CARE, MEDICARE-CERTIFIED HOME HEALTH CARE AND NON-MEDICAL HOME CARE SERVICES. MOREOVER, WE SUPPORT FAMILY CAREGIVERS THROUGH COMPLIMENTARY EDUCATIONAL OFFERINGS, CONNECTIONS TO COMMUNITY RESOURCES, AND NEARLY 250,000 HOURS OF RESPITE CARE (NUMBER IMPACTED BY ONSET OF COVID-19 PANDEMIC). PLEASE SEE SCHEDULE R FOR RELATED ENTITIES. FSL'S CHIEF OF OPERATIONS, CARRIE SMITH, WAS HONORED BY THE PHOENIX BUSINESS JOURNAL WITH AN OUTSTANDING WOMEN IN BUSINESS AWARD. FSL MADE SIGNIFICANT ADVANCEMENTS IN ITS INFORMATION TECHNOLOGY INFRASTRUCTURE WITH A MIGRATION IN PAYROLL SERVICES TO CERIDIAN/DAYFORCE AND A NEW PERFORMANCE MANAGEMENT SOLUTION, PROPHIX. A NEW CHIEF FINANCIAL OFFICER, TERESA CELAYA-GARNER, JOINED THE TEAM IN APRIL 2020. THE COVID-19 PANDEMIC CAUSED FSL TO CANCEL ITS ANNUAL FUNDRAISER AND CLOSE ITS THREE ADULT DAY HEALTH SERVICES CENTERS FOR THE FINAL QUARTER OF FY 2019-20 (APRIL 1 JUNE 30). FSL SAW A 45% INCREASE IN DEMAND FOR HOME-DELIVERED MEALS. TO MEET COMMUNITY NEED, FSL CREATED A FREE GROCERY BAG DELIVERY PROGRAM WHICH PROVIDED HOME-DELIVERED ESSENTIAL FOOD, PERSONAL HYGIENE, AND PAPER SUPPLIES TO SENIORS IN NEED. THE ORGANIZATION MADE THE DIFFICULT DECISION TO LAY OFF A THIRD OF ITS EMPLOYEES BUT WAS APPROVED FOR A PAYCHECK PROTECTION PROGRAM LOAN AND WAS ABLE TO BRING STAFF BACK AFTER ONLY ONE MISSED PAYCHECK. DUE TO FSL'S REPUTATION AND STRONG RELATIONSHIPS WITH COMMUNITY FOUNDATIONS AND CORPORATIONS, FSL WAS ABLE TO RAISE NEARLY $1,000,000 IN EMERGENCY RELIEF FUNDING.
