Organizations Filed Purposes: THE MISSION OF THE SOUTHWESTERN CT AGENCY ON AGING IS TO PROVIDE LEADERSHIP AND RESOURCES TO MEET THE NEEDS OF THE RAPIDLY GROWING OLDER ADULT POPULATION AND TO MAINTAIN AND IMPROVE THE QUALITY OF LIFE AND INDEPENDENCE OF OLDER PERSONS AND PERSONS WITH DISABILITIES.

NUTRITION AND SOCIAL SERVICES: THE ORGANIZATION IS A DESIGNATED AREA AGENCY ON AGING AND ADMINISTERS STATE AND OLDER AMERICANS ACT FEDERAL ELDERLY FUNDS TO SUPPORT THE NEEDS OF OLDER ADULTS IN THE 14-TOWN REGION OF SOUTH-WESTERN CONNECTICUT. FUNDS SUPPORT THE INDEPENDENCE OF OLDER ADULTS BY PROVING IN-HOME AND COMMUNITY-BASED MEALS, EVIDENCE-BASED HEALTH PROMOTION, CAREGIVER SUPPORTS, AND INFORMATION AND REFERRAL ASSISTANCE PROGRAMS. THE ORGANIZATION IS RESPONSIBLE TO WORK WITH AN ADVISORY COUNCIL TO DEVELOP A THREE-YEAR PLAN AND SERVE AS AN ADVOCATE FOR OLDER INDIVIDUALS WHO WISH TO LIVE IN THEIR COMMUNITIES WITH MAXIMUM INDEPENDENCE.

HOME CARE: THE ORGANIZATION IS A CONTRACTOR FOR THE CT DEPARTMENT OF SOCIAL SERVICE'S MEDICAID WAIVER. THE ORGANIZATION IS RESPONSIBLE FOR DOCUMENTING THE ASSESSMENT OF POTENTIAL CLIENTS' FUNCTIONAL AND FINANCIAL ELIGIBILITY FOR THE CONNECTICUT HOME CARE PROGRAMS. STAFF DEVELOP A COMPREHENSIVE, PERSON-CENTERED CARE PLAN TO MEET THE CLIENT'S ONGOING NEEDS. UPON AN ELIGIBILITY DECISION FROM THE DEPARTMENT OF SOCIAL SERVICES, THE ORGANIZATION PROVIDES ONGOING CASE MANAGEMENT, MONITORING, ANNUAL REDETERMINATION AND ASSISTANCE DESIGNED TO GIVE CLIENTS AN ALTERNATIVE TO NURSING HOME INSTITUTIONALIZATION. THE STAFF AUTHORIZE AND DOCUMENT PRIOR AUTHORIZATIONS THAT VALIDATE THE PAYMENT OF THE DIRECT-SERVICE CONTRACTORS WHO PROVIDE IN-HOME CLIENT SUPPORTS.
