CORE Provide Counseling, Independent Living Skills and Advocacy to 295 consumers with disabilities to promote independence, productivity and quality of life in the areas of employment, housing, transportation, transition from nursing facilities to community living, education, among other goal areas. Provide information and referral to 1547 people with disabilities, their families, other service providers and the general public.
HOPWA and PART B Assisted 73 individuals and family members with disabilities to achieve their independent living goals by offering financial rental assistance, housing subsidies and purchase of goods and services that support community based living.
Money Follows the Person Aging and Disability Resource Centers Assisted people with disabilities and elders to prevent unnecessary placement in nursing facilities, including transition of 38 people living in nursing homes to community based living. Also provided services to 28 elder veterans to obtain benefits and maintain community living.
