Organizations Filed Purposes: Core Services-Individuals with diverse disability categories were provided with services to help increase their independence in their community. During FYE9/30/20 there were over 199 new or continued individuals served. There were 10 individuals who received assistive technology services 7 individuals who received communication services 76 received general advocacy services 118 independent living and life skills training and recreational service referrals to local or national businesses and agencies, 47 peer counseling and family services to provide a person centered approach, 12 preventative care, and physical restoration services, 59 vocational referrals, 13 received transportation assistance and mobility training, 22 received house/home modifications/shelter services, there were 5 individuals receiving youth/transition services and 1 received family services. These core services are vital to increasing independence for individuals with disabilities in their community, schools and/or places of employment. CRP Community Rehabilitation Provider - Assist individuals with visible and invisible disabilities in obtaining and maintaining competitive employment. Services include, but are not limited to, evaluations within real world work settings that are tailored based on the consumers strengths, weaknesses, and disability barriers fostering exchange of information with local businesses, creating professional and social contacts to assist with developing job sites for individuals provide one-on-one mentoring to employed individuals who need the supports to learn or maintain skills related to their job Educate individuals on interviewing skills, techniques and interpersonal skills. During the reporting year, vocational services were provided one-on-one to 59 individuals.

Case Services-During the FYE9/30/20, there were 44 consumers that received financial support from Part B Case Service Funds and/or Restricted Funds received from donors. These funds were used to remove or modify barriers allowing the consumer to live and/or work more independently in their homes and workplace. These modifications may include ramps, grab bars, repairs and access to accessible transp., air conditioner for consumers with breathing disability, assist with a consumers move auto repair to maintain employment transp. to necessary appointments wheelchair lift for vehicle enabling mobility to a consumer the purchase of hearing aids batteries for hard of hearing and deaf safety equip. such as vibrating alarm clocks, flashing doorbells smoke detectors for deaf hard of hearing individuals, computer appropriate software for blind consumers utility bills rent for consumer laid off and facing eviction home repairs such as water heater, bed for consumer sleeping on floor, and winter clothing for consumer who lost everything while in the hosp. due to her disability.
