COMMUNITY OF HOPE WORKS TO IMPROVE HEALTH AND END FAMILY HOMELESSNESS TO MAKE WASHINGTON, DC MORE EQUITABLE.
HOUSING FOR FAMILIES EXPERIENCING HOMELESSNESS - COH WORKS TO END HOMELESSNESS FOR FAMILIES BY OFFERING SERVICES AT ALL LEVELS OF THE HOMELESSNESS CONTINUUM OF CARE. IN 2019, WE SERVED 1,228 FAMILIES, INCLUDING 3,960 PEOPLE. WE OFFER HOMELESSNESS PREVENTION SERVICES THAT ASSISTED 653 FAMILIES, AND 90% OF THE CASES DID NOT END UP IN SHELTER. IN 2019, WE PROVIDED TEMPORARY HOUSING AT GIRARD STREET (WARD 1) FOR 30 FAMILIES WITH CHILDREN AND SERVED 169 FAMILIES AT THE TRIUMPH, OUR SHORT-TERM FAMILY HOUSING SITE FOR UP TO 50 FAMILIES. WE ALSO PROVIDED TRANSITIONAL HOUSING FOR 13 FAMILIES AT HOPE APARTMENTS (WARD 8) WHERE THE HEAD OF HOUSEHOLD IS RECOVERING FROM SUBSTANCE ABUSE IN 2019. ADDITIONALLY, WE USE TWO PERMANENT HOUSING SOLUTIONS: PERMANENT SUPPORTIVE HOUSING (PSH) FOR CHRONICALLY HOMELESS FAMILIES; AND RAPID RE-HOUSING (RRH) FOR FAMILIES WITH LOWER BARRIERS TO HOUSING STABILITY, BUT WHO STILL NEED SHORT-TERM RENTAL ASSISTANCE AND SUPPORT. OUR PSH PROGRAMS ARE MISSISSIPPI AVENUE APARTMENTS AND ARCHER PARK (WARD 8), OUR SITE-BASED PERMANENT SUPPORTIVE HOUSING PROGRAM, HOUSES 18 HIGHLY VULNERABLE FAMILIES AND 11 INDIVIDUALS AND IN LATE 2019, WE ALSO BEGAN SERVING 11 HOUSEHOLDS AT DC HOUSING AUTHORITY'S PARKWAY OVERLOOK. HOME NOW, OUR LARGEST PSH PROGRAM, PROVIDES SCATTERED-SITE HOUSING ACROSS THE CITY TO 168 FAMILIES AT A TIME. 319 FAMILIES WERE SERVED IN OUR SCATTERED-SITE RRH PROGRAM IN 2019. SUPPORTIVE SERVICES FOR FAMILIES INCLUDE CASE MANAGEMENT, EMPLOYMENT SERVICES, MENTORING, LIFE SKILLS CLASSES, AND ACTIVITIES FOR THE CHILDREN. PSH HAD A SUCCESS RATE OF 100% IN ENDING HOMELESSNESS FOR FAMILIES. RRH HAS KEPT 87% OF ITS FAMILIES FROM RETURNING TO SHELTER WITHIN TWO YEARS OF PROGRAM EXIT.
HEALTH SERVICES - COH WORKS TO IMPROVE HEALTH OUTCOMES IN WASHINGTON, DC, BY PROVIDING QUALITY, AFFORDABLE AND ACCESSIBLE PRIMARY HEALTHCARE WITH A FOCUS ON SERVING THE UNINSURED AND UNDERINSURED IN OUR COMMUNITY. AS A FEDERALLY QUALIFIED HEALTH CENTER (FQHC), COH PROVIDES MEDICAL, DENTAL, AND BEHAVIORAL HEALTH CARE AT THREE HEALTH CENTERS AND BEHAVIORAL HEALTH AT A COMMUNITY RESOURCE HUB TO ALL PERSONS REGARDLESS OF THEIR ABILITY TO PAY. IN 2019, WE PROVIDED CARE TO 13,835 PATIENTS IN OVER 60,000 MEDICAL, DENTAL, BEHAVIORAL AND CARE COORDINATION VISITS. OUR LARGEST HEALTH CENTER, CONWAY HEALTH AND RESOURCE CENTER (CHRC) PROVIDES MEDICAL, DENTAL, AND BEHAVIORAL HEALTH SERVICES IN WARD 8, FILLING A CRITICAL GAP IN ONE OF THE MOST UNDERSERVED AND IMPOVERISHED AREAS OF DC. WE ALSO PROVIDE A RANGE OF HEALTH SERVICES AT OUR MARIE REED HEALTH CENTER (MRHC) IN WARD 1. AT OUR FAMILY HEALTH AND BIRTH CENTER (FHBC), LOCATED IN WARD 5, WE PROVIDED PRIMARY MEDICAL CARE AND BEHAVIORAL HEALTHCARE. AT THIS SITE AND CHRC, WE ALSO CONTINUED TO STRENGTHEN AND EXPAND SERVICES TO PREGNANT WOMEN AND THEIR FAMILIES, FROM THE PRENATAL THROUGH POSTPARTUM STAGES. FOR EXAMPLE, WE OFFER THE CENTERINGPREGNANCY PROGRAM, A GROUP PRENATAL CARE APPROACH THAT EMPOWERS PREGNANT WOMEN TO TAKE CHARGE OF THEIR OWN HEALTH AND THE HEALTH OF THEIR BABIES, AT FHBC AND CHRC. IN 2019, WE PURCHASED A NEW HOME FOR FHBC, LOCATED A MILE AND A HALF FROM OUR CURRENT LOCATION, AND PLAN TO GREATLY EXPAND OUR SERVICES AT THE NEW CENTER WHEN IT OPENS IN 2022 AFTER RENOVATIONS AND CONSTRUCTION IS COMPLETE. IN 2019, WE CONTINUED TO RAMP UP OUR EMOTIONAL WELLNESS SERVICES, INCLUDING AT THE COMMONS AT STANTON SQUARE, OUR SATELLITE SITE IN WARD 8 WHICH OPENED IN 2018. AT THE COMMONS, IN ADDITION TO A WIDE RANGE OF THERAPY SERVICES, WE OFFER HOMELESSNESS PREVENTION, AND OTHER FAMILY SUPPORTS AND PARTNER WITH OTHER ORGANIZATIONS FOR FOOD ACCESS AND EARLY CHILDHOOD EDUCATION. WE ALSO BEGAN OFFERING SCHOOL-BASED MENTAL HEALTH SERVICES AT THREE HIGH-NEED ELEMENTARY SCHOOLS IN WARD 8 IN 2019.OUR HEALTH CENTERS ARE PATIENT-CENTERED MEDICAL HOMES. WE PROVIDE A RANGE OF PATIENT SUPPORT SERVICES, INCLUDING, FOR EXAMPLE, A HOME VISITING PROGRAM FOR FAMILIES WITH YOUNG CHILDREN. THROUGH THIS PROGRAM, CASE MANAGERS EDUCATE PARENTS ABOUT INFANTS AND TODDLERS' PHYSICAL AND EMOTIONAL DEVELOPMENT AND CONNECT FAMILIES TO RESOURCES. WE HAVE CONTINUED TO PROVIDE A ROBUST CARE COORDINATION PROGRAM CALLED MY HEALTH GPS TO SUPPORT PATIENTS WITH MULTIPLE CHRONIC DISEASES. WE ALSO OFFER CARE COORDINATION SERVICES AND NUTRITION FOR WOMEN AGES 18-45 WHO OFTEN NEGLECT THEIR HEALTH CARE AS THEY PROVIDE FOR THEIR FAMILIES, AMONG OTHER SERVICES. COH CONTINUES TO SERVE A LARGELY LOW-INCOME POPULATION. 84% OF PATIENTS EARN AT OR BELOW 200% OF THE FEDERAL POVERTY LINE. 90% HAVE MEDICAID, MEDICARE, OR DC ALLIANCE, OR ARE UNINSURED AND QUALIFY FOR DISCOUNTED FEES. 8% OF OUR PATIENTS ARE BEST SERVED IN A LANGUAGE OTHER THAN ENGLISH, AND 93% ARE OF RACIAL OR ETHNIC MINORITY. IN DECEMBER 2019, WE WERE CHOSEN TO SERVE AS THE BELLEVUE FAMILY SUCCESS CENTER BY THE CHILD AND FAMILY SERVICES ADMINISTRATION. FAMILY SUCCESS CENTERS ARE PART OF A NEW CITY-WIDE PLAN CALLED FAMILIES FIRST DC. WITH THIS NEW PLACE-BASED APPROACH, WE HOPE TO EMPOWER COMMUNITIES, INTEGRATE SERVICES, AND PROVIDE UPSTREAM SUPPORT.IN 2019, WE ALSO COMPLETED OUR 2020-2024 STRATEGIC PLAN IN WHICH WE REFRESHED OUR MISSION, VISION, AND ORGANIZATIONAL GOALS. THIS PLAN WILL GUIDE OUR WORK FOR THE COMING FEW YEARS.
