ADVANCE AN EFFECTIVE AND EFFICIENT HEALTH SYSTEM BUILT ON A STRONG FOUNDATION OF PRIMARY CARE AND THE PATIENT-CENTERED MEDICAL HOME.

PRACTICE TRANSFORMATION - WITH SUPPORT FROM THE CENTERS FOR MEDICARE AND MEDICAID SERVICES (CMS), THE TRANSFORMING CLINICAL PRACTICE INITIATIVE (TCPI) IS DESIGNED TO ASSIST MORE THAN 140,000 CLINICIAN PRACTICES FROM 2015-2019 IN SHARING, ADAPTING, AND FURTHER DEVELOPING OF COMPREHENSIVE QUALITY IMPROVEMENT STRATEGIES. AS A TCPI AWARDEE, PCC WORKS TO SUPPORT PRACTICE IMPROVEMENT TEAMS THOUGH ITS DIVERSE NETWORK OF STAKEHOLDERS TO FOSTER PARTNERSHIPS WITH PATIENTS, FAMILY CAREGIVERS, AND COMMUNITY-BASED ORGANIZATIONS TO ACHIEVE COMMON GOALS OF IMPROVED CARE, BETTER HEALTH AND REDUCED COSTS.

ANNUAL CONFERENCE - HOLD AN ANNUAL CONFERENCE TO ADDRESS KEY POLICIES IN ELEVATING PRIMARY CARE THROUGH EDUCATIONAL SESSIONS AND IN-DEPTH DISCUSSIONS ON HOW TO MAKE THE VISION PRIMARY CARE SHOULD BE PERSON-CENTERED, TEAM-BASED, AND COMMUNITY-ALIGNED A REALITY.

MEMBER SERVICES - PROVIDE MEMBER SUPPORT THROUGH ENGAGING IN POLICY AND ADVOCACY EFFORTS TO ALLOCATE THE RESOURCES NEEDED FOR ADVANCED PRIMARY CARE AND ENSURE CONSUMER ACCESS TO SUCH CARE; SPONSORING AFFINITY GROUPS, POLICY CONVENINGS, AND MEMBERSHIP-ENGAGEMENT EVENTS; AND INFORMING MEMBERS ABOUT PAYMENT, DELIVERY, AND BENEFIT INNOVATIONS, AS WELL AS OTHER EVIDENCE, RELATED TO ADVANCED PRIMARY CARE THROUGH WEBINARS, PUBLICATIONS, NEWSLETTERS, AND PCC'S WEBSITE
