Organizations Filed Purposes: MISSION: THE MISSION OF THE PACIFIC CANCER FOUNDATION IS TO PROVIDE ACCESS, KNOWLEDGE AND SUPPORT TO ALL THOSE AFFECTED BY CANCER IN MAUI COUNTY. WE DO THIS BY PROVIDING FREE SUPPORT SERVICES TO MAUI'S CANCER COMMUNITY. PURPOSE: THE PACIFIC CANCER FOUNDATION IS AN INDEPENDENT 501 (C)(3) NOT-FOR-PROFIT ORGANIZATION CREATED EXCLUSIVELY FOR CHARITABLE AND EDUCATIONAL PURPOSES. THROUGH THE IMPLEMENTATION OF VARIOUS PROGRAMS, THE FOUNDATION GIVES MEANINGFUL ASSISTANCE TO CANCER PATIENTS AND THEIR FAMILIES. THE FOUNDATION WORKS TO FOSTER PUBLIC AWARENESS OF THE NEEDS OF CANCER PATIENTS. THE FOUNDATION SERVICES THE COMMUNITY BY PROVIDING A CONNECTION BETWEEN THE CANCER PATIENT, SURVIVOR AND CAREGIVER AND THE RESOURCES THEY NEED WHETHER THEY BE SOCIAL, FINANCIAL, MENTAL, SPIRITUAL OR PHYSICAL. THE FOUNDATION SUPPORTS CARE THAT IMPROVES THE QUALITY OF LIFE FOR CANCER PATIENTS AND SURVIVORS. HISTORY: THE PACIFIC CANCER FOUNDATION WAS CONCEIVED IN 2004 BY CONCERNED HEALTH CARE PROVIDERS AND CANCE

THE GOAL OF THE PACIFIC CANCER FOUNDATION IS TO PROVIDE FREE SUPPORT SERVICES TO MAUI COUNTY'S CANCER COMMUNITY. WE ASSIST PATIENTS AND THEIR FAMILIES BY PROVIDING ACCESS, KNOWLEDGE AND SUPPORT, WHICH WILL BE BENEFICIAL IN DEALING WITH THEIR DISEASE. ACCESS: THE FOUNDATION SEEKS TO PROVIDE EASY AND EFFECTIVE ACCESS TO SERVICES FOR CANCER PATIENTS. THE FOUNDATION PROVIDES TRANSPORTATION SERVICES SO CLIENTS CAN ACCESS TESTS, TREATMENT, SECOND OPINIONS AND SPECIALISTS. THE FOUNDATION SUPPORTS CARE THAT IMPROVES THE QUALITY OF LIFE FOR CANCER PATIENTS. KNOWLEDGE: THE FOUNDATION DISTILLS THE VAST AMOUNT OF INFORMATION AVAILABLE ON CANCER IN ORDER TO PROVIDE MANAGEABLE EDUCATIONAL RESOURCES FOR PATIENTS, THEIR FAMILIES AND HEALTHCARE PROVIDERS WISHING TO KNOW MORE ABOUT DISEASE. THE FOUNDATION GENERATES PUBLIC AWARENESS AS WELL AS SUPPORT AND UNDERSTANDING OF THE NEEDS OF THE CANCER PATIENTS AND THEIR FAMILIES. THE FOUNDATION WILL SERVE AS AN ADVOCATE FOR PATIENTS AND THEIR FAMILIES IN ORDER TO ENSURE THAT THEY RECEIVE THE UTMOST RESPECT AND DIGNITY AS THEY FACE THE CHALLENGES OF THEIR DISEASE. SUPPORT: THE FOUNDATION IDENTIFIES FOR THE CANCER PATIENT AND THEIR FAMILIES SUPPORT GROUPS AND SPECIAL EVENTS SPECIFIC TO THEIR NEEDS. THE FOUNDATION PROVIDES A RESOURCE CENTER THAT PROVIDES PERSONAL ASSISTANCE FOR THOSE DESIRING INFORMATION ON THE DISEASE.
