HealthLINC Incorporated ("HealthLINC") serves medical providers and institutions throughout Indiana by supporting health information technology ("HIT") adoption, health information exchange ("HIE"), and innovative use of information for improved health care outcomes. HealthLINC enables healthcare providers to make smarter decisions that enhance care, improve outcomes, enhance efficiencies, and lower healthcare costs. HealthLINC achieves these goals with its clients by eliminating duplicate medical testing, providing a secure environment for providers to share patient information, and providing real time electronic delivery of lab, radiology, and transcribed reports. HealthLINC provides the following services: Care Notification Service ("CNS") Quality care often depends on knowing where patients have received care. Hospital visits cost significantly more than a regular office visit, and result in fragmented care, medication confusion, and missed opportunities for preventive and routine examination. Timely follow-up and collaboration on transitions can improve patient outcomes. With daily notification, physicians and other clinicians can accelerate their patients follow-up care in the office following ED visits, outpatient procedures, or hospitalizations. CNS notifies physicians and other clinicians when their patients are admitted to the emergency or other hospital department, receive outpatient procedures, are admitted or discharged from the hospital, or experience a significant change in Medication or Re-Admission Risk Score. Community Medication Management ("CMM") A transition from one care setting to another is a vulnerable time for patients. Many medication errors occur during these transitions. The CMM tool aggregates multiple medical lists into one shared location allowing healthcare professionals to better understand a patients' medication history and use that information to make a difference. CMM is a web-based application that allows providers from multiple care settings to impart vital information about their shared patients in a secure environment. CMM has a variety of tools to assist in medication management. Automated medication feeds are updated daily or in some cases real-time. Those medications are displayed in a comparison chart, which sorts medication by therapeutic class, grouping medications that are identical, similar and unique into an easy to read format, allowing easy identification of errors, potential drug therapy problems, and deprescription opportunities. Conferences HealthLINC has held annual conferences for many years. Its conferences are excellent ways for medical providers and their staff to learn about the latest developments in HIT and Practice Transformation. DIRECT Secure Email DIRECT Secure Email is a secure, encrypted web-based communication system for physicians, nurse practitioners, physician assistants, and other healthcare providers to share protected health information with providers outside of their organization. E-mail, fax, or regular mail involves the inherent risks of information being compromised or accessed by unauthorized users. Direct messaging is HIPAA-compliant and mitigates these risks by providing an easy way for health care providers to securely share patient information electronically with providers who are outside of their internal system. Great Lakes Practice Transformation Network ("GLTPN") The GLTPN helps approximately 11,500 clinicians prepare for anticipated industry changes calling for better care at lower costs. HealthLINC's networked, five-state coalition is dedicated to coaching clinicians through the five phases of patient-centric practice transformation necessary to effectively participate in value-based payment systems. HealthLINC guides clinicians through the various CMS phases by utilizing a Transformational Change Toolkit to help them achieve associated milestones as well as clinical operational results. IU Health Medical Records Remote Access Support As a trusted partner of IU Health, HealthLINC provides support to users outside of the IU Health System with electronic access to medical records from many facilities within the IU Health System. Giving providers access to their patients' most current testing results allows providers to effectively diagnose, reduce medical errors, and provide efficient care. Results Delivery HB/messaging is an effective way for hospitals, radiology groups, and independent labs to share patient results with physician offices. It is an electronic system that securely delivers test reports. For practices wanting to receive results, HealthLINC provides either a "direct to your EMR interface", or a web-based clinical application called hb/messaging. Once a clinical test is resulted or report transcribed, it will automatically populate your inbox. By accessing your inbox you will be able to view and print your results. HealthLINC delivers lab, radiology, and transcriptions for your provider(s) from any of the hospitals, radiology centers, or labs that participant in our HIE. It also has the ability to filter results that providers feel are non-relevant to your daily workflow.
