Organizations Filed Purposes: SWIRCA IS A PUBLIC SERVICE ORGANIZATION THAT FOCUSES ITS EFFORTS ON EMPOWERING SENIORS AND PEOPLE WITH DISABILITIES TO REMAIN LIVING SAFELY IN THEIR OWN HOMES BY PROVIDING INFORMATION AND SUPPORTIVE SERVICES.

THE SOUTHWESTERN INDIANA REGIONAL COUNCIL ON AGING (SWIRCA & MORE) IS A NOT FOR PROFIT ORGANIZATION WHOSE MISSION IS ENHANCING OPPORTUNITIES FOR INDEPENDENT LIVING. SWIRCA & MORE ASSISTS OLDER ADULTS AND INDIVIDUALS WHO ARE DISABLED OF ALL AGES THROUGHOUT A SIX COUNTY AREA INCLUDING GIBSON, PERRY, POSEY, SPENCER, VANDERBURGH, AND WARRICK. SWIRCA & MORE HAS DEVELOPED A COMPREHENSIVE ARRAY OF SERVICES INTENDED TO PREVENT INSTITUTIONALIZATION AND FOSTER A SAFE HOME ENVIRONMENT FOR EACH CLIENT WE SERVE.

CASE MANAGEMENT-SWIRCA & MORE CARE MANAGERS PROVIDED SOME SORT OF ASSISTANCE TO 6,154 UNDUPLICATED CLIENTS BETWEEN 07/01/2019 THROUGH 06/30/2020. THE TYPES OF ASSISTANCE PROVIDED BY A CARE MANAGER MAY INCLUDE ELIGIBILITY DETERMINATIONS FOR NURSING HOME PLACEMENT, ELIGIBILITY DETERMINATIONS FOR HOME AND COMMUNITY BASED SERVICES, GENERAL ADVOCACY, RESOURCE LINKAGE, AND/OR ON-GOING MONITORING OF CLIENTS RECEIVING FUNDING FOR IN-HOME SERVICES THROUGH ANY OF SWIRCA'S FUNDING SOURCES. ON THE LATTER POINT REGARDING INDIVIDUALS WHO RECEIVED IN-HOME SERVICES, OF THE 3,720 INDIVIDUALS WHO RECEIVED ASSISTANCE FROM A CARE MANAGER, 1,706 INDIVIDUALS RECEIVED DIRECT IN-HOME ASSISTANCE FROM AT LEAST ONE OF SWIRCA'S FIVE FUNDING SOURCES (AD WAIVER, CHOICE, SSBG, TITLE III-E, OR TITLE III). THE TYPES OF IN-HOME SERVICES PROVIDED THROUGH THESE PAYERS INCLUDE, BUT ARE NOT LIMITED TO, CASE MANAGEMENT, ATTENDANT CARE, HOMEMAKER ASSISTANCE, ADULT DAY SERVICES, ASSISTED LIVING, HOME HEALTH SERVICES, DURABLE MEDICAL EQUIPMENT OR SUPPLIES, HANDYMAN, HOME DELIVERED MEALS, OR RESPITE SERVICES. ALSO, 55 INDIVIDUALS RECEIVED HOME MODIFICATIONS TO MAKE THEIR HOME MORE ACCESSIBLE AND MET THE NEEDS OF THEIR DISABILITY. MODIFICATION MAY INCLUDE BUILDING A RAMP, INSTALLING A LIFT OR MODIFYING A BATHROOM TO INCREASE THE CLIENTS INDEPENDENCE. SWIRCA & MORE IS ALSO ONE OF FOUR CONTRACTEES WITH THE INDIANA DIVISION OF AGING TO ADMINISTER THE MONEY FOLLOWS THE PERSON (MFP) PROGRAM. THE GOAL OF MFP IS TO ASSIST INDIVIDUALS WHO ARE RESIDING IN NURSING FACILITIES TO TRANSITION BACK TO LIVING IN A COMMUNITY SETTING. MONEY FOLLOWS THE PERSON PROGRAM ENDED WITHIN FY2019. WE ARE FOLLOWING THE REST OF THE INDIVIDUALS RECEIVING THAT FUNDING SOURCE AS THEY TRANSITION HOME AND ONTO OTHER FUNDING SOURCES. WE WILL CONTINUE PROVIDING CMGT SERVICES TO THOSE RECEIPIENTS.

NUTRITION-THE SWIRCA & MORE NUTRITION PROGRAMS PROVIDED DAILY HOME DELIVERED MEALS THROUGHOUT SOUTHERN INDIANA COUNTRIES BY WAY OF MEAL ROUTES. THE MEAL RECIPIENTS INCLUDE CLIENTS WHO ARE HOMEBOUND AND UNABLE TO PREPARE A HOT NUTRITIOUS LUNCH AS WELL AS CARE GIVERS OF ELDERLY FAMILY MEMBERS WHO ARE UNABLE TO TRAVEL TO A SWIRCA & MORE CONGREGATE MEAL SITE SUPPORTING OUR COMMUNITY MEMBERS BY PROVIDING DAILY WELL-BALANCED MEALS FOR THOSE IN NEED.

WELLNESS AND COMMUNITY PROGRAMS-SWIRCA & MORE HEALTHY AGING/WELLNESS PROGRAM SERVES AN AVERAGE OF 3,363 PEOPLE PER MONTH (DUPLICATED). WITH AN AVERAGE OF 164.06 PER DAY (DUPLICATED) VISITING THE ACTIVITY CENTER. THERE ARE A VARIETY OF PROGRAMS OFFERED WHICH INCLUDE THE FOLLOWING: YOGA, TAI CHI, ENHANCED FITNESS, ARTHRITIS EXERCISE CLASS, STRETCH & TONE, LINE DANCING, EDUCATIONAL HEALTH CLASSES (WITH HEALTH VOLUNTEERS FROM STUDENT NURSES, PHYSICAL AND OCCUPATIONAL THERAPISTS FROM USI AND UE), EXERCISE ROOM, COMMUNITY HEALTHY AGING SPEAKERS, SOCIAL ACTIVITIES (SPECIAL EVENTS/PARTIES, BINGO, POKENO, CRAFTS, MOVIE DAYS, VOLUNTEER ACTIVITIES, BOOK CLUB, COMMUNITY OUTINGS). THE SWIRCA & MORE ACTIVITY CENTER ALSO HOSTS THE ANNUAL NURSING HOME AND ASSISTED LIVING GAMES IN ALL 6 COUNTIES.
