Organizations Filed Purposes: OUR MISSION IS TO PROVIDE OPPORTUNITIES THAT ADVANCE THE EDUCATION, HEALTH, WELL-BEING AND INDEPENDENCE FOR NEAR-SOUTHSIDE FAMILIES. WE OFFER A VARIETY OF ACTIVITIES TO NEIGHBORHOOD RESIDENTS OF ALL AGES, COLLABORATES WITH OTHER AGENCIES TO PROVIDE DECENTRALIZED OR SPECIAL SERVICES FOR THE COMMUNITY, AND SERVES AS A FACILITATOR FOR THE NEAR SOUTHSIDE INDIANAPOLIS COMMUNITY IN ADDRESSING THEIR NEEDS AND INTERESTS AS WELL AS SUPPORTING THEIR VALUES IN PROCURING APPROPRIATE RESOURCES.

CHILDREN AND YOUTH DEVELOPMENT EARLY LEARNING CONCORD PROVIDES A NATIONALLY ACCREDITED, PATHS TO QUALITY IV EARLY LEARNING PROGRAM FOR CHILDREN AGES 3-5 YEARS. THE PROGRAM SEEKS TO BUILD FOUNDATIONAL SKILLS AND PREPARE STUDENTS FOR SUCCESS IN SCHOOL. THE LEAD TEACHERS ARE CDA CERTIFIED, AND THEY USE THE CREATIVE CURRICULUM TO TEACH I-STAR KR STANDARDS. THE PROGRAM IS A FULL-DAY MODEL TO SUPPORT THE CHILDCARE NEEDS OF FAMILIES. IN 2019, THE EARLY LEARNING PROGRAM ENROLLED 63 STUDENTS, AND 100% OF GRADUATES TESTED "KINDERGARTEN READY." YOUTH DEVELOPMENT, NOW FOR THE FUTURE, AND EXTRA-CURRICULAR TEEN DEVELOPMENT YOUTH DEVELOPMENT PROGRAMS ARE DESIGNED TO FOLLOW SCHOOL-AGED YOUTH THROUGH COMPLETION OF THEIR CHOSEN POST-SECONDARY ENDEAVOR. IN ELEMENTARY SCHOOL, THE PROGRAM OFFERS A VARIETY OF EDUCATION, HEALTH, SOCIAL, AND RECREATIONAL ACTIVITIES. NOW FOR THE FUTURE IS THE MIDDLE SCHOOL/TEEN PROGRAM, AND ITS SCHEDULE FURTHER FOCUSES ON COLLEGE-AND-CAREER READINESS AND INTERPERSONAL SKILLS DEVELOPMENT. PROGRAMMING IS OFFERED BEFORE- AND AFTER-SCHOOL AND FULL-DAYS WHEN SCHOOL IS NOT IN SESSION. CONCORD ALSO OFFERS A TWICE-WEEKLY EXTRA-CURRICULAR RECREATION PROGRAM "TEEN NIGHT." IN 2018, CONCORD PROVIDED QUALITY PROGRAMS AND SUPERVISION FOR 249 YOUTH.

HIV CARE COORDINATION CONCORD IS AN ISDH CARE COORDINATION PROGRAM SITE. THE PROGRAM SERVES INDIVIDUALS LIVING WITH AN HIV+ DIAGNOSIS TO BEST MANAGE THEIR HEALTH AND OVERALL QUALITY OF LIFE. CLIENTS RECEIVE ASSISTANCE WITH MEDICAL, PSYCHOSOCIAL, HEALTHCARE NEEDS, INSURANCE ENROLLMENT, AND GENERAL BASIC NEEDS ASSISTANCE. IN 2019, 205 CLIENTS ACCESSED THE CARE COORDINATION PROGRAM AT CONCORD.

NUTRITION CONCORD STRIVES TO IMPROVE LOCAL HEALTH OUTCOMES BY SERVING QUALITY, FRESH AND HEALTHY MEALS TO PROGRAM PARTICIPANTS AND RESIDENTS. THE DYNKI DELI CONCORD OPERATES AN ON-SITE CAFE DURING THE WEEKDAY LUNCH HOURS. THE DYNKI DELI IS A LOW-COST, HEALTHY OPTION FOR RESIDENTS AND SENIOR PROGRAM PARTICIPANTS THAT PREVENTS HUNGER AND PROVIDES NUTRITIONAL MEALS. THE MENU IS COMPRISED OF ROTATING DAILY SPECIALS AND TRADITIONAL DELI ENTREES. SENIOR MEALS ARE SUBSIDIZED. THE DYNKI SERVED 3,007 MEALS IN 2019. CHILD FOOD SERVICES CONCORD MAINTAINS A FEDERAL GRANT THROUGH CACFP THAT SUPPORTS DAILY SNACKS AND LUNCHES FOR EARLY LEARNING AND YOUTH DEVELOPMENT PROGRAM PARTICIPANTS. CONCORD'S NUTRITION PROGRAM IS MANAGED ON-SITE, AND IT ADHERES TO INDUSTRY BEST-PRACTICES AND QUALITY STANDARDS. CHILD FOOD SERVICES ENSURES THAT YOUTH PARTICIPANTS ARE NOT HUNGRY, AND THAT THEY ARE DEVELOPING HEALTHY EATING HABITS EARLY. ALL MEALS INCLUDE AT LEAST ONE FRESH FRUIT OR VEGETABLE. THE CHILD FOOD SERVICES PROGRAM PROVIDED 14,638 MEALS IN 2019.
