Methodist Medical Group, Inc. ("Methodist Medical Group") was dissolved on December 31, 2017. Prior to its dissolution, Methodist Medical Group was the sole member of MH Healthcare, Inc. ("MH Healthcare"). MH Healthcare was also dissolved on December 31, 2017. Prior to MH Healthcare's dissolution, it was a member of Rehabilitation Hospital of Indiana, Inc. ("RHI"), a 501(c)(3) tax-exempt organization under IRC 170(b)(1)(A)(iii). After MH Healthcare's dissolution, its former membership in RHI transferred to Indiana University Health, Inc. ("IU Health"). RHI's mission is to provide personalized, compassionate care that enables individuals to regain hope and independence after a life-changing injury or illness. RHI is the only rehabilitation hospital in the State of Indiana that is CARF accredited in brain injury, spinal cord injury, and stroke. Through Methodist Medical Group's sole membership in MH Healthcare, and MH Healthcare's membership in RHI, Methodist Medical Group effectively had the ability to appoint two out of the four members of RHI's board of directors. This ability gave Methodist Medical Group a significant voice in the governance, operations, and program services of RHI.
Prior to Methodist Medical Group's dissolution on December 31, 2017, it was a 1% member in Health Venture Management, LLC ("Health Venture Management"), a management company that provides services to ambulatory surgery centers that are majority-owned by Methodist Medical Group's sole member, IU Health. Methodist Medical Group considered this to be a minor activity in comparison to its primary program service accomplishments discussed in Line 4a above. Immediately prior to Methodist Medical Group's dissolution on December 31, 2017, its 1% member interest in Health Venture Management, LLC was transferred to Indiana University Health Ball Memorial Hospital, Inc.
