The mission of Community of Care is to "care for people by partnering with rural communities." Our goal is "to enhance the quality of life for rural Cass residents.
One Stop Service Center and Faith Community Nurse Program. Personal and professional assistance with a wide variety of needs and concerns, including: in-home service support, financial issues, equipment needs, housing, transportation, nutrition, Medicare Part D drug plan enrollment, exercise programs, health education, health support, and care coordination. Through information, outreach, exercise programs, blood pressure screenings, referrals, education, health coordination, advance care plans and care coordination, staff provides assistance for client concerns. Staff include Care Coordination, a Masters in Social Work and trained North Dakota State Health Insurance Counselor through the North Dakota Insurance Department and the Faith Community Nurse, a licensed RN who has completed Faith Community Nurse training. Both employees are trained to assist in completing Advance Care Plans. During 2019, Part d open enrollment, 267 clients received assistance. The total savings for these individuals was $79,223 or an average of $296/client. Four trained SHIC counselors assisted the Care Coordinator in completing Part D appointments. Care Coordinator facilitated socialization events for older adults in rural Cass County during the summer of 2019. The Faith Community Nurse Program coordinated Bone Builder exercise classes in six communities serving 95 older adults for bi-weekly classes. Through these two programs, we provided 5,025 client contacts in 2018.
Volunteer Program. Staff facilitate and coordinate services to assist clients remain in their rural homes as long as safely possible. Trained volunteers provide these services: transportation to medical appointments, social transportation, minor home repair, yard work, assistance with errands and friendly visits. In 2019, we collaborated with local businesses for the United Way of Cass Clay "Day of Caring." In 2019, fifty volunteers provided 28,264 miles and 1,497 hours of services providing 445 rides. Volunteers (exercise class, committees, fundraisers, office assistance, college interns, etc.) provided an additional 1,213 hours of service and drove 5,662 miles. The board of directors provided 397 hours of services and 3,889 miles serving the organization. In total, one hundred and twenty-three volunteers provided 3,107 hours of service in 2019. The economic impact of our volunteer services in 2019 was $92,247.
Support and Education - we provided the following services for local residents: care coordination health monitoring, blood pressure screenings, caregiver support, socialization activities, health related education events, presentations to local organizations, and newsletter (mailed twice a year to all rural Cass County box holders - approximately 7,200). In 2019 we coordinated with the Casselton Business Association to sponsor Sr. Day. Programming included "Health Living for Your Brain and Body" by a representative from the Alzheimer's Association, "What to know about the Census?" by a Cass County Census Representative and "Medicare Advantage Plans" by a local pharmacist and our Care Coordinator. Displays represented organizations providing services to older adults. The Health Cabinet sponsored three "Fun with Health" events to provide information on caregiving We sponsored "Estate Planning SMARTS" in 2019 with presentation by the only certified elder attorney in North Dakota. In 2019, staff gave presentation to approximately 775 individuals. In 2019, staff provided 5,758 duplicated visits and/or services for 519 individuals. Clients from 26 rural Cass County communities were served in 2019.
