EASTMONT TOWERS IS A FULL CONTINUUM PROVIDER. MANY EASTMONT RESIDENTS ENTER INTO A LIFE CARE CONTRACT FOR THEIR COMPLETE CARE WHEN THEY MOVE INTO INDEPENDENT LIVING APARTMENTS. THERE ARE 121 APARTMENTS LOCATED IN THREE DIFFERENT BUILDINGS AND APARTMENTS ARE RENOVATED WITH QUALITY CONSTRUCTION. LIVING INDEPENDENTLY AT EASTMONT INCLUDES A MEAL PLAN, TRANSPORTATION TO SHOPPING AND MEDICAL APPOINTMENTS, BI-WEEKLY HOUSEKEEPING, MAINTENANCE, AND A RICH PROGRAM AND EVENT SCHEDULE. RESIDENTS AT EASTMONT HAVE ACCESS TO 24-HOUR NURSING AND CAN SEE THE APARTMENT RN'S IN THE CLINICS MONDAY - FRIDAY. THE EASTMONT CHAPLAIN ASSISTS WITH SPIRITUAL NEEDS AND EASTMONT PROVIDES MANY OPPORTUNITIES FOR SPIRITUAL FULFILLMENT. THE HEALTH CARE TEAM ASSISTS INDEPENDENT RESIDENTS WHEN THEIR NEEDS CHANGE AND HEALTH CARE SERVICES BECOME NECESSARY.
EASTMONT TOWERS PROVIDES ASSISTED LIVING FOR THOSE INDIVIDUALS NEEDING ASSISTANCE WITH ACTIVITIES OF DAILY LIVING. THE TWO AREAS ON THE MAIN CAMPUS LICENSED AS ASSISTED LIVING (THE SEASONS AND THE LIVE-IN CENTER) HAVE A TOTAL OF 69 BEDS AND USE AN ACTIVITY-DRIVEN SOCIAL MODEL WITH AN EMPHASIS ON FELLOWSHIP, ENRICHMENT, AND EDUCATIONAL OPPORTUNITIES. RESIDENTS HAVE ACCESS TO LICENSED PROFESSIONAL NURSING STAFF ON CAMPUS 24 HOURS PER DAY. INDIVIDUALS HAVE TAILORED PROGRAMS THAT CAN INCLUDE ASSISTANCE WITH ACTIVITIES OF DAILY LIVING, MEDICATION MANAGEMENT, HEALTH MAINTENANCE ACTIVITIES, TRANSFERS OR AMBULATION, OXYGEN, AND ADDITIONAL COMPANION OR HOME HEALTH NEEDS.
THE EASTMONT TOWERS CARE CENTER HAS A TOTAL OF 42 BEDS. OF THESE BEDS, 20 ARE LICENSED FOR MEDICARE REHABILITATION AND 22 ARE DESIGNATED FOR LONG-TERM CARE. THE CARE CENTER PROVIDES SKILLED NURSING, PHYSICAL, OCCUPATIONAL, SPEECH AND ACTIVITY THERAPY IN AN INTERDISCIPLINARY ENVIRONMENT. ADDITIONALLY, SPIRITUAL GUIDANCE, SOCIAL WORK, CASE MANAGEMENT, AND CARE SERVICES ARE PROVIDED FOR THE OVERALL PSYCHOSOCIAL WELL-BEING OF THE PATIENTS. THE CARE CENTER IS GUIDED BY A MEDICAL DIRECTOR, LICENSED NURSING HOME ADMINISTRATOR, AND DIRECTOR OF NURSING.
