THE ACT PROGRAM FOCUSING ON INDIVIDUALS WITH COMPLEX MENTAL HEALTH NEEDS AND A HISTORY OF HOSPITALIZATIONS ASSISTED INDIVIDUALS IN ACHIEVING 96% COMMUNITY TENURE (IE. DAYS OUTSIDE OF THE HOSPITAL). 74% OF INDIVIDUALS IN RESIDENTIAL REHABILITATION SERVICES TRANSITIONED TO MORE INDEPENDENT LIVING AT DISCHARGE.

EMPLOYMENT AND COMMUNITY SERVICES: HOUSING, JOB AND COMMUNITY SUPPORTS WHEN AND WHERE ONE NEEDS IT. 7 OUT OF 10 PERSONS WITHOUT ANY SOURCE OF INCOME OBTAINED RESOURCES THROUGH EMPLOYMENT, FEDERAL OR STATE ASSISTANCE PROGRAMS, OR OTHER MEANS. 56% OF ALL SSI/SSDI APPLICATIONS SUBMITTED USING THE SOAR MODEL WERE APPROVED COMPARED TO THE 28.5% NATIONAL AVERAGE. 98% OF INDIVIDUALS SERVED IN HOMELESS TRANSITION MAINTAINED HOUSING PLACEMENTS DURING THE YEAR.

FAMILY AND PEER SUPPORT: PRACTICAL, HANDS-ON ADVICE AND UNDERSTANDING FROM OTHERS WHO HAVE BEEN THERE. 98% OF ON-SITE GUESTS CREDITED THE PEER STAFFED CRISIS DIVERSION SERVICES WITH PREVENTING AN EMERGENCY ROOM VISIT OR HOSPITALIZATION. THE SAFE HARBOR PEER CRISIS DIVERSION PROGRAM HAD OVER 500 INDIVIDUAL VISITS AND ANSWERED OVER 12,000 CALLS. 197 PERSONS RECEIVED FAMILY EDUCATION SERVICES.
